关节镜下应用Endopearl与Intrafix固定四股半腱肌腱重建膝关节后交叉韧带 被引量:10

Arthroscopic Reconstruction of the Posterior Cruciate Ligament with Semitendinosus Fixed with EndoPearl,Interference Screw and Intrafix

摘要 目的探讨关节镜下应用Endopearl与Intrafix固定四股半腱肌腱重建膝关节后交叉韧带的近期临床效果。方法取自体的双侧半腱肌腱对折成四束后对12例单纯后交叉韧带损伤(新鲜损伤5例,陈旧性损伤7例;Ⅱ度损伤4例,Ⅲ度损伤8例)行关节镜下重建,移植肌腱股骨侧用Endopearl加可吸收界面螺钉固定,胫骨侧用Imrafix固定。结果12例均获得随访,随访时间7~14个月(平均8.5个月);术后3个月膝关节活动范围均超过120°,无伸膝受限,后抽屉试验Ⅰ度阳性2例,其余为阴性;Lyshlom评分从术前(85.55±2.68)分增加至术后(90.6±2.3)分。结论关节镜下应用Endopearl与Imrafix固定四股半腱肌腱单束重建膝关节后交叉韧带方法确切,可有效恢复膝关节功能。 Objective To study the early stage effectiveness of arthroscopic reconstruction of posterior cruciate ligament (PCL) using semitendinosus fixed with the EndoPearl, the interference screws and the Intrafix. Methods Twelve patients with simple PCL injury (including 5 patients with fresh fracture and 7 with:old fracture; Ⅱ degree in 4 patients, Ⅲ degree in 8 patients) underwent the PCL arthroscopic reconstruction using quadrisemitendinosus autograft and the tendons were fixed with EndoPearl and interference screws at the femoral side and with Intrafix at the tibial side. Results All patients were followed up from 7 months to 14 months, averaging 8.5 months. No extension limitation occurred and the flexion was more than 120°in all patients 3 months postoperatively. The posterior drawer test was negative in all but 2 patients who had Ⅰ degree instability. The Lysholm knee score increased from 85.55 ± 2.68 preoperatively to 90.6 ± 2.3 postoperatively. Conclusion Arthroscopic reconstruction of the posterior cruciate ligament with semitendinosus fixed by EndoPearl and Intrafix is effective and reliable for the restoration of knee function.
