摘要
目的应用^(99)Tc^m-甲氧基异丁基异腈(MIBI)/^(18)F-脱氧葡萄糖(FDG)心肌灌注断层显像及代谢显像评价经皮冠状动脉腔内成形术(PTCA)及支架置入的效果。方法 33例冠心病患者在成功的 PTCA 及支架置入术前及术后(平均)3个月进行心肌灌注/代谢显像。结果术前、术后患者心肌缺血节段数分别为2.7±0.4与1.7±0.3(P<0.05);心肌细胞代谢低下节段数分别为1.9±0.3与1.2±0.3(P<0.05)。术后较术前心肌缺血及心肌细胞代谢低下节段平均减少数分别为0.98±0.2与0.65±0.2(P<0.05)。结论成功的 PTCA 及支架置入术后心肌缺血和心肌细胞代谢明显改善,但心肌细胞代谢改善程度远不如血流灌注。
Objective To investigate the efficiency of percutaneous transluminal coronary angio- plasty( PTCA ) and coronary stent using ^99Tc^m-methoxyisobutylisonitrile ( MIBI ) /^19F-fluorodeoxyglucose (FDG) SPECTimaging in coronary heart disease. Methods ^99Tc^m-MIBI myocardial perfusion and metabolic imaging were performed in 33 patients before and after PTCA and(or) coronary stent. Results After PTCA and (or) coronary stent, the number of myocardial perfusion defects and hypo-metabolic segments sig- nificantly decreased from 2.7 ± 0.4 to 1.7 ± 0.3 ( P 〈 0.05 ) and 1.9 ± 0.3 to 1.2 ± 0.3 ( P 〈 0.05 ), respectively. The average decline segments myocardial defects and with hypo-metabolic segments were 0.98 ± 0.2 and 0.65 ± 0.2(P 〈 0.05 ). Conclusion The myocardial perfusion and cell metabolism can be both improved significantly in successful PTCA and coronary stent, but the degree of change in cell metabolism is far less than the perfusion.
