摘要
目的:探讨凋亡抑制基因survivin与核因子-κB(nuclearfactor kappa B,NF-kB)在骨肿瘤中的表达及临床意义.方法:采用免疫组化染色方法检测了105例骨肿瘤(其中骨肉瘤85例、骨软骨瘤20例)中survivin与NF-κBp65蛋白的表达,并对骨肉瘤主要临床资料和病理分级参数进行比较,用χ2检验进行统计学处理.结果:在骨肉瘤中survivin与NF-κB65蛋白阳性表达率分别为75.3%、63.5%,而在骨软骨瘤中的阳性表达率均为0.0%.survivin与NF-κBp65蛋白在骨肉瘤临床分期中Ⅱb期、Ⅲ期的表达率明显高于Ⅰ、Ⅱa期(P<0.001).病理分级survivin与NF-κBp65蛋白阳性表达率分别为Ⅰ级45.0%、35.0%,Ⅱ级73.5%、58.8%,Ⅲ级96.8%、87.1%,随着恶性程度增高而显著增加(P<0.001).有软组织浸润和转移者survivin与NF-κBp65蛋白阳性表达率明显高于非浸润和非转移组(P<0.001~0.005).NF-κB p65和survivin同时表达者87.0%(47/54),明显高于NF-κB P65阴性表达中survivin阳性表达54.8%(17/31),两组有明显差异(P<0.05).结论:凋亡抑制基因survivin与NF-κB p65蛋白在骨肉瘤中不同程度异常表达与肿瘤的临床分期、病理分级和浸润、转移呈正相关,提示survivin与NF-κB p65蛋白的高表达可能与骨肉瘤的发生发展和转移密切相关.
Objective:To investigate the expressions of survivin and NF-kB on osteosarcoma and discuss their significance. Methods:The expressions of survivin and NF-kB protein in 105 patients consisting of 85 cases with osteosarcoma and 20 osteochondroma were detected by Immunohistochemistry. The main clinical data and pathological classification parameters in osteosarcoma were analyzed and statistically treated with chi-square test. Result: The positive rates of survivin and NF-kB proteins in osteosarcoma cases were 75.3 % and 63.5 % respectively, both positive rates were obviously higher than in osteochondroma cases. The expression of survivin and NF-kB proteins were higher in clinical stage Ⅱb and Ⅲ than in stage Ⅰ and Iia (P 〈0.001). The expression of survivin and NF-kB proteins in grade Ⅰ ,Ⅱ and Ⅲ were 35.0% and 35.0% ,61.8% and 58,8% ,and 90, 3% and 87.1% respectively, the increased levels were along with malignant degree of osteochondroma (P 〈 0.001 ). The expressions of both proteins were higher in cases of invasion and metastasis than cases without those ( P 〈 0.001-0.005), Among NF-kB(+) patients,87.0% of cases simultaneously expressed surviving,the ratio was higher than that in NF-kB(-) group (41.9% ),the difference was significant (P 〈0. 05). Conclusion: Abnormal expression of survivin, which is an apoptosis suppressor gene, and NF-kB in osteosarcoma is direct correlation with clinical stage, pathological grades and invasion and metastasis. The increased expressions of survivin and NF-kB perhaps correlate to the occurrence, progression and metastasis of osteosarcoma.
