三种颈前路钢板重建对维持颈椎曲度和高度的比较 被引量:3

COMPARISON OF RESTORING AND MAINTAINING THE CERVICAL CURVATURE AND HEIGHT BY USING THREE DIFFERENT ANTERIOR CERVICAL PLATE SYSTEMS

摘要 目的 对3种不同颈椎前路钢板系统重建并维持颈椎融合节段曲度和高度的作用进行比较。方法2002年1月~2004年6月,行颈前路减压,自体髂骨植骨,钢板内固定术122例;男85例,女37例,年龄14~70岁。根据采用Orion、Zephir、Codman3种颈椎前路钢板系统分为A组37例、B组39例及C组46例。于术前、术后1周和随访时摄颈椎侧位X线片,测量融合节段的曲度和高度,并比较其变化及3组间差异。结果术后患者均获随访6~35个月,平均17.3个月。术后6个月3组患者术段颈椎均获骨性融合。且术后1周时颈椎融合节段曲度和高度较术前明显改善(P〈0.05),但最后1次随访时和术后1周比较,差异无统计学意义(P〉0.05),3组间比较差异无统计学意义(P〉0.05)。结论3种钢板系统均能有效重建并维持颈椎融合节段曲度和高度,近期效果优良。 Objective To investiga curvature and height of the fused segment by January 2002 to June 2004, 122 patients unde e and compare the effects of restoring and maintaining the cervical using three different anterior cervical plate systems. Methods From went anterior cervical decompression, autogenous iliac bone graft and plate fixation. Of the 122 patients (85 males, 37 females, aged 14-70), 37 underwent surgery involving the fixation with the Orion plate system, 39 with the Zephir plate system, and 46 with the Codman plate system. The cervical curvature and height of the fused segment were measured on the lateral X-ray films so as to compare the changes of the conditions preoperatively, 1 week after surgery, and during the follow-up, and also to compare the difference among the three groups. Results The follow-up of the patients for 6-35 months (average 17.3 months) showed that all the patients developed the bone fusion 6 months after operation. There was a significant improvement in the cervical curvature and height of the fused segment before operation versus 1 week after operation(P〈0. 05); however, there were no significant changes 1 week after operation versus during the follow-up in each group(P〉0. 05); there was no significant difference among the three groups (P〉0. 05 ). Conclusion The three plate systems can effectively reconstruct and maintain the cervical curvature and height of the fused segment, with a satisfactory effect in a short term.
