摘要
目的:研究氟哌啶醇和氯丙嗪对慢性精神分裂症患者的糖脂代谢和体重的影响。方法:对87例服用氟哌啶醇和83例服用氯丙嗪的患者于治疗前后不同时间作血糖、胰岛素、血脂及体重测定,并作相关因素分析。结果:服药90,180d后氟哌啶醇组糖尿病发生率分别为1.1%和2.3%,氯丙嗪组分别为1.1%及3.6%。与治疗前相比,氯丙嗪组90d时空腹和餐后2h血糖升高,180d时进一步升高;氟哌啶醇组不明显。氯丙嗪组90d时体重较基线增高平均为4.8%;180d为7.4%;氟哌啶醇组不明显。胰岛素两组均增加(P<0.05),但组间无差别(P>0.01)。胆固醇和三酰甘油检查,氯丙嗪组均增高(P<0.01),氟哌啶醇组无变化。氯丙嗪组的血药浓度与2h血糖相关,r=0.64(P<0.01)。结论:氯丙嗪对糖脂代谢和体重有影响。建议在用药期间进行监测。
OBJECTIVE To explore the impact of haloperidol and ehlorpromazine treatment to the body weight and metabolism of glucose and lipid in chronic schizophrenia patients. METHODS 87 cases with haloperidol and 83 cases with chlorpromazinc were recruited. Weight, blood glucose, insulin, cholesterol and triglyceride were determined in baseline,90 and 180 days of treatment. RESULTS The incidence of diabetes after 90 days and 180 days of halperidol group the incidence are 1.1% and 2.3%, for 90 days and 180 days of treatment and in chlorpromazine group are 1.1% and 3.6%. Compared with baseline, the level of blood glucose of fast and 2 hours after meal in chlorpromazine was elevated after 90 days of treatment and even higher after 180 days of treatment. The 4evel of blood glucose in ehlorpromazine group is higher than other groups. After 90 days treatment,the weight increased 4. 8% in chlorpromazine group. After 180 days the weight increased 7. 4% in chlorpromazine group. The level of insulin increased in three groups after treatment (P〈0. 05), but there is no significant difference between the two groups. The level of blood cholesterol and triglyceride increased in chlorpromazine group after treatment (P〈0.01 ), but did not in haloperidol group(P〉0.01 ). There are some correlations in the change of blood glucose, insulin, cholesterol, triglyceride and body weight r = 0. 64(P〈0.01 ). CONCLUSION Chlorpromazine definitely affects the body weight and metabolism of glucose and lipid in chronic schizophrenia patients, It is better to monitor these indexs.
