摘要
目的了解老年颈动脉粥样硬化患者肱动脉内皮依赖性舒张功能(即血流介导的血管扩张功能,flow-mediateddilation,FMD)、硝酸甘油介导的非内皮依赖性舒张功能(nitroglycerin-mediateddilation,NMD)、血浆血管内皮活性因子一氧化氮(NO)、内皮素(ET)的水平以及普罗布考对上述指标及血清总胆固醇(TC)水平的影响。方法分别于口服普罗布考(0.5g,每日2次,持续12周)前后检测43例老年颈动脉粥样硬化患者肱动脉的FMD及NMD;同时测定NO、ET和TC水平,并与40例健康老人进行对照研究。结果老年颈动脉粥样硬化患者肱动脉的FMD及NMD均显著低于健康老年组(P<0.05),NO水平显著低于健康老年组,而ET却显著高于健康老年组;老年颈动脉粥样硬化组口服普罗布考12周后的FMD和NMD较治疗前显著升高(P<0.05),TC水平显著降低(P<0.05),NO水平显著升高,ET水平显著降低(P<0.05)。结论(1)老年颈动脉粥样硬化患者肱动脉FMD及NMD均受损;(2)老年颈动脉粥样硬化患者存在血管内皮功能失调;(3)普罗布考能降低老年颈动脉粥样硬化患者的胆固醇水平,改善血管内皮功能。
Objective To evaluate the vascular endothelial function of the old patients with carotid atherosclerosis and the effect of probueol on the vascular endothelial function and the total cholesterol level. Methods Before and after oral administration of probucol ,the endothelium-dependent ( i. e. flow-mediated dilation, FMD ) and endothelium-independent dilation( i. e. nitroglycerin-mediated dilation, NMD)of the 43 old patients with carotid atherosclerosis were detected and compared with 40 old healthy controls. Meanwhile, the plasma level of nitrix oxide(NO) and endothelin(ET) was examined Results ( 1 ) Both FMD and NMD of the brachial artery in the old patients with carotid atherosclerosis were less than those of the controls significantly (P 〈 0. 05 ). (2)The plasma concentration of NO of the old patients with carotid atherosclerosis was lower than that of the controls, but the ET level was higher. ( 3 ) After 12-week treatment of probucol, the cholesterol level reduced and the vascular endothelial function improved. Conclusions ( 1 ) Both FMD and NMD of the old patients with carotid atherosclerosis are impaired. (2) Dysfunction of vascular endothelium exists in the old patients with carotid atherosclerosis. ( 3 ) Probucol can decrease the cholesterol level and ameliorate the endothelial function of the old patients with carotid atherosclerosis.
