摘要
目的研究发育和行为的预见性保健指导在婴幼儿保健门诊的应用。方法儿科门诊行健康体检的42d足月婴儿126例,随机分为发育和行为的预见性保健指导组(指导组)64例和常规保健组(常规组)62例。指导组由儿童保健医师对其家长进行婴幼儿发育和行为的预见性保健指导培训,并由家长对其孩子加以实施;2组婴幼儿均行常规保健指导。2组婴幼儿于生后9,12,18,24个月采用中国儿童发展中心婴幼儿智力量表(CDCC)行发育商测试,并行体格发育等级评定。结果指导组于生后9,12,18和24个月时智力发育指数(MDI)及心理运动发育指数(PDI)高于常规组;体格发育主要指标(身长、体质量)高于常规组,2组比较差别有统计学意义(P<0.05)。结论发育和行为的预见性保健指导可促进婴幼儿行为、智能发育及体格发育;能早期发现婴幼儿发育和行为的偏离情况,便于早期干预,降低偏离率。将发育和行为的预见性保健指导纳入儿童保健门诊常规工作是可行和有效的。
Objective To explore a service mode for the infants' health care clinic in the new century, anticipative development and behavior intervension to the infants were applied in the clinic. Methods 42 days healthy termed infants who were examined in health care clinic were divided into two groups randomly; routine(n=62) and instruction group(n=64). The parents of instruction group were trained the ways of anticipative development and behavior intervension by the health-keepers and took practice in the activities comparing to the routine normal health care. The physical development and intelligence were checked regularly by infants' intelligence scale of Chinese children development center (CDCC) on the 9th, 12th, 18th and 24th month after born. Results On the specific months, the mental development index(MDI) and psych-motet development index(PDI) in intervension group were higher than those in the routine(P〈0. 05). The body weight and length of physical development, were higher than those in the control too(P〈0. 05). Conclusion Applications of anticipative development and behavior instruction can improve the infants ' intellectual-physical growth and early-developments. It is operative and effective to apply anticipative development and behavior instruction in the infants' health care clinic.
