摘要
目的:通过观察血脂、脂质过氧化指标及血浆内皮素、降钙素基因相关肽、血栓素B2、前列腺素F1α水平的改变,分析复方马齿苋对冠状动脉粥样硬化性心脏病(冠心病)伴高黏血症的临床干预作用。方法:选择2002-04/05在潍坊医学院附属医院中医科及潍坊市人民医院保健科就诊的冠心病伴高黏血症患者120例,均自愿参加观察。按1∶1比例随机分为两组,复方马齿苋组、对照组各60例。复方马齿苋组口服复方马齿苋冲剂,3次/d,两袋/次(每袋10g,相当于生药15g);对照组口服地奥心血康胶囊200mg,3次/d,两组疗程均为42d(6周)。治疗前及治疗6周后观察两组患者各相关指标并进行对比:①总胆固醇、三酰甘油采用酶法测定,高、低密度脂蛋白胆固醇采用比浊法测定。②血清脂质过氧化物含量和超氧化物歧化酶活性。③内皮素、降钙素基因相关肽、血栓素B2、前列腺素F1α采用放射免疫法测定。④观察治疗前后两组患者临床表现。结果:纳入120例患者,全部进入结果分析,无脱落。①与对照组比较,用药6周后复方马齿苋组总胆固醇及低密度脂蛋白胆固醇水平显著降低(P<0.05,0.01),而高密度脂蛋白胆固醇水平显著升高(P<0.05);血清脂质过氧化物含量显著降低,超氧化物歧化酶活性显著升高(P<0.01);内皮素、血栓素B2含量及血栓素B2/前列腺素F1α比值降低,而降钙素基因相关肽、前列腺素F1α含量显著升高(P<0.05~0.01)。②用药6周后马齿苋组患者在胸闷、头晕、气短方面的总有效率均优于对照组(χ2=4.39~18.01,P<0.05~0.01)。结论:复方马齿苋具有良好的调脂、抗氧化及血管内皮细胞保护作用,对冠心病伴高黏血症患者的临床干预作用显著。
AIM: To investigate the clinical effects of compound purslae on coronary atheroselerotic heart disease (CAHD) complicated with hyperviscosity through observing the changes of blood lipids, lipid peroxidation, plama endothelins, calcitonin-gene-relaed peptide (CGRP), thromboxane B2(TXB2) and prostagladin F1α (PGF1α.
METHODS: Totally 120 patients with CAHD complicated with hyperviscosity were selected from the Department of Traditional Chinese Medicine of Affiliated Hospital of Weifang Medical College and Department of Health Care of Weifang People's Hospital from April to May 2002. According to the ratio of hl, volunteers were divided randomly into compound purslae group and control group with 60 in each group, Patients in compound phrslae group were treated with compound purslae, three time/day, two bag/time, (10 g/bag, equal to raw drug 15 g). Patients in control group were treated with 200 mg di'ao xinxue kang, three time/day. They were treated for 6 weeks. Correlaed indexes of patients in the two groups were compared before and 6 weeks after treatment: ① Levels of total cholesterol (TC) and triacylglycerol (TG) were assayed with enzyme method, and levels of high density lipoprotein (HDL) and low density lipoprotein (LDL) were assayed with nephelometer; ② Contents of serum lipid peroxidation (LPO) and superoxide dismutase (SOD) were measured with reagent box; ③ Levels of endothelins, CGRP, TXB2 and PGF1αwere measured with radio-immunity method. ④ Clinical situation was observed before and after treatment.
RESULTS: Totally 120 patients entered the final analysis without any loss. ① After 6 weeks, levels of TC and LDL were decreased in compound purslae group as compared with those in control group (P 〈 0.05, 0.01); level of HDL was increased obviously (P 〈 0.05); content of plama lipid peroxidation was decreased, and SOD activity was increased (P 〈 0.01); contents of endothelins and TXB2 and ratio of TXB2/PGF1αwere decreased obviously; contents of CGRP and PGF1αwere increased (P 〈 0.05-0.01). ② After 6 weeks, total effective rates of chest stuffy, dizzy, breathe hard in compound purslae group were superior to those in control group (X^2=4.39-18.01, P 〈 0.05-0.01).
CONCLUSION: Compound purslae has a obvious effect on regulating lipid, antioxidition ,and protecting vessel endothelial cell in the patients with CAHD complicated with hyperviscosity.
