紫外线阻断型及蓝光滤过型人工晶状体植入术后临床研究

摘要 目的比较研究紫外线阻断型(AcrySof)及蓝光滤过型(AcrySof Natural)人工晶状体(intraocularlens,IOL)植入术后患者主观感觉、最佳矫正视力(best corrected visual acuity,BCVA)、对比敏感度(contrastsensitivity function,CSF)和色觉的变化。方法采用前瞻性、随机、双盲的研究方法。老年性白内障患者60人(62眼),随机分成两组,分别植入 AcrySof IOL32眼、AcrySof Natural IOL30眼。术后6个月问卷调查患者主观感觉、检查 BCVA、CSF 和色觉。结果植入 AcrySof Natural IOL 的患者主观感觉较好。所有患者术后 BCVA、CSF均较术前明显提高,差异有非常显著性意义(P<0.01)。在所有空间频率,两组 IOL 植入术后 CSF 差异无显著性(P>0.05)。两组术后色觉检查差异不显著。结论 AcrySof Natural IOL 中加入黄色载色基团是安全有效的。AcrySof 与 AcrySof Natural IOL 植入术后患者视力、CSF 和色觉差异没有显著性。尚需进行长期临床随访以确定AcrySof Natural IOL 是否能降低老年性黄斑变性进展的危险性。 Purpose To investigate the difference in subjective sensation , best corrected visual acuity (BCVA), contrast sensitivity function (CSF) and color vision after UV-blocking (AcrySof) and blue-light filtering ( AcrySof Natural ) intraocular lens(IOL) implantation. Methods A prospective, randomized, double-blindness clinical trial was conducted. Sixty senile cataract patients (62 eyes) were divided into two groups in random. AcrySof IOLs were implanted in 32 eyes and AcrySof Natural IOLs in 30 eyes. Subjective sensation , BCVA, CSF and FM-100 color vision were recorded at 6 months postoperatively, respectively. Results The subjective sensation after AcrySof Natural IOLs implantation was better than AcrySof IOLs. BCVA and CSF improved in all cases postoperatively, and there was very significant difference (P 〈 0.01 ) . Both CSF and color vision were similar between AcrySof and AcrySof Natural IOLs implantation postoperatively. AcrySof Natural IOL implantation was safe and effective. BCVA, CSF and color vision were similar after AcrySof and AcrySof Natural IOL implantation, long term follow up was needed to confirm whether AcrySof Natural IOL implantation can decrease the incidence of age related macular degeneration.
