摘要
目的:建立急性肾功能衰竭(ARF)动物模型并研究多层螺旋CT灌注成像在ARF中的应用价值。材料和方法:兔后腿肌注50%甘油等渗盐水10ml/kg,建立急性肾小管坏死性ARF动物模型,分别于注射前、注射后6h、24h、48h共四个时间点行CT灌注扫描。运用GE公司Perfusion2软件,测量各时间点肾皮质的CT灌注平均值,包括血流量(BF)和血容量(BV),进行统计学分析,并将测量结果与病理结果进行对比分析。结果:6h时间点肾皮质BF、BV值明显降低,与正常者相比有显著性差异(P<0.01),24h有所上升,48h时BF和用药前平均值无显著性意义(P=0.205),BV有显著差异(P=0.041)。6h时肾脏光镜下病理改变不明显,24h后始出现典型改变,48h更为显著。结论:兔急性肾小管坏死性ARF时,血流动力学的改变先于典型病理改变出现,CT灌注成像能较准确地评价血流动力学的改变。
Purpose: To establish an experimental animal model of acute renal fallure(ARF) in rabbits and investigate the applicative value of CT perfusion imaging in them. Materials and Methods: The model of ARF was established in rabbits through the intramuscular injection of 50% glycerin (10ml/ kg) into the hind leg, The multisection dynamic CT perfusion examination was performed for right kidney on the day before and hours 6, 24, 48 after the injection. The mean BF, BV were measured in renal cortex using Perfusion 2 software package(GE) and statistically analyzed, respectively, Then a correlative analysis was conducted between BF, BV and pathological changes in kidney cortex, Results: The significant decrease of BF, BV of renal cortex appeared at 6h( P 〈 0,01), No significant change in BF was found at 48h (P = 0.205), however, remained abnormal still in BV(P = 0.041) , Under light microscope, pathological changes occurred at 24h, remarkabled at 48h. Conclusion: the preliminary study shows that CT perfusion may be useful and very promising for evaluation of ARF in the early stage.
