松果体区病变的MRI和^1H-MRS表现与病理相对照研究 被引量:3

MRI and ^1H - MRS on Diagnosis of Pineal Region Disease - Correlative to Pathological Findings

摘要 目的:分析松果体区病变MRI表现并探索1H-MRS及DWI对于鉴别松果体区病变的作用。材料和方法:松果体区病变患者共33例,其中男27例,女6例,年龄7~51岁,平均年龄21.3±10.3岁。全部33例都作了MRISE-T1WI,FSE-T2WI,PDWI,FLAIR及增强MRI成像检查。其中7例患者术前行1H-MRS和DWI检查。结果:大多松果体区病变有其特征的外形、信号强度及强化方式;MRS:2例生殖细胞瘤Ch/NAA(2.06和2.11)、LL/Cr(0.83和4.2);2例成熟畸胎瘤LL/Cr(7.3和9.64),测不到Ch和NAA;2例星形细胞瘤Ch/NAA(3.55和3.53);1例松果体母细胞瘤Ch/NAA4.70。ADC值:生殖细胞瘤7.48×10-4和6.48×10-4;畸胎瘤2.03×10-3和3.57×10-3;星形细胞瘤1.91×10-3和1.35×10-3;松果体母细胞瘤9.14×10-4。结论:病变的外形特征、信号强度及强化方式对于松果体区病变的诊断最有帮助。1H-MRS及DWI在某种程度上对于鉴别诊断有帮助。 Purpose: To analyze characteristic MR Imaging of pineal region diseases and to explore the value of DWI and ^1H- MRS in differential diagnosis. Materials and Methods: 33 patients with pineal region diseases proved by pathology presented at 7 to 51 years old (mean 21.3 - 10.3 Y.O.), 6 were females and 27 males.All of them underwent MRI SE - T1WI, FSE - T2WI, PDWI, FLAIR and post - contrasted MR scan. Furthermore, MRS and DWI before operation were achieved in seven patients. Results: Most pineal region diseases have their tipical morphology, singal intensity and enhancement style. On MRS, germinoma showed Ch/NAA (2.06 and 2.11), LL/Cr (0.83 and 4.2), mature teratomas LL/Cr(7.3 and 9.64), without Ch and NAA and astrocytoma Ch/ NAA (3.55 and 3.93); pineoblastoma Ch/ NAA 4.7. ADC value of germinoma was 7.48×10^-4 and 6.48×10^-4 ; mature teratoma 2.03×10^-.3 and 3.57×10^-3; astrocytoma 1.91 ×10^-3 and 1.35 ×10^-3 ; pineoblastoma 9.14 ×10^-4 . Conclusion: Change in morphologic features, singal intensity and enhancement style is the most helpful variable for the differential diagnosis of pineal region diseases. To some extent DWI and ^1H - MRS imaging enable distinction among them.
