纤维粘连蛋白mRNA表达量与乳腺癌转移的关系及临床意义

摘要 目的:探讨乳腺癌纤维粘连蛋白(Fibronectin,FN)mRNA表达量与乳腺癌转移的关系及临床意义。方法:采用荧光定量RT-PCR方法,检测30例乳腺原发癌和配对淋巴结转移癌以及116例乳腺原发癌的FNmRNA表达量,比较30例乳腺原发癌和淋巴结转移癌的FNmRNA表达量,分析FNmRNA表达量与乳腺癌临床病理因素的关系。结果:30例乳腺淋巴结转移癌FNmRNA表达量低于原发癌(P<0.05)。肿瘤﹥5cm的FNmRNA表达量低于肿瘤≤5cm(P<0.05),临床期的FNmRNA表达量低于Ⅰ期和Ⅱ期(P<0.05),淋巴结4枚以上阳性FNmRNA表达量低于1~3枚阳性(P<0.05),雌、孕激素受体阳性的FNmRNA表达量高于雌、孕激素受体阴性。FNmRNA表达量与病理类型、组织学分级、Her-2表达、CA153表达无关。结论:FNmRNA表达量在临床早期及淋巴结转移早期上调,随着肿瘤的进展表达量下调。FNmRNA转录水平的表达与乳腺癌转移有关。 Objective: To discuss the clinical fibronectin mRNA and metastasis of breast cancer. significance and relationship between expression of Methods: The fibronectin mRNA expression of 30 cases with paring lymph-node metastatic carcinoma and 116 with primary breast cancer were detected and the relationship between the fibronectin mRNA expression and clinicopathologic factors of breast cancer was analyzed using fluorescent- quantitative RT-PCR. Results: FN mRNA expression in 30 metastatic lymphnode was lower than in primary breast cancer (P〈0.05). The mRNA expression of FN in the tumor over 5cm was lower than in those less or equal to 5cm (P〈0.05). The FN mRNA expressions in stage Ⅰ and were lower than in stage Ⅱ (P〈0.05). The mRNA expression of FN in more than 4 lymph nodes positive was lower than in lymph node 1-3 positive (P〈0.05). The mRNA expressions of FN in ER and PR positive were higher than in ER and PR negative. The FN mRNA expression was irrelated with tissue grade, pathologic type, Her-2 and CA153. Conclusion: FN mRNA expression is up-regulated in the clinical early stage of the lesion and the early stage of lymph-node metastasis, and is down-regulated with the progression of tumors. FN mRNA expression is related with the metastasis of breast cancer.
