摘要
目的观察子宫全切举宫器在腹腔镜下全子宫切术中或腹腔镜辅助下的阴式子宫切术中的应用价值。方法回顾性分析腹腔镜全子宫切术或腹腔镜辅助下阴式子宫切除术49例根据不同手术方法分为2组,Ⅰ组应用子宫全切举宫器29例;Ⅱ组应用阴道内填塞纱布20例。观察两组的手术时间、术中出血量及并发症的情况。结果Ⅰ组的手术时间(100.86±25.04)min比Ⅱ组(116.5±27.68)min明显缩短(p<0.05),术中出血量Ⅰ组(147.24±48.76)ml比Ⅱ组(179.5±62.87)ml明显减少(P<0.05),Ⅰ组无1例并发症,Ⅱ组出现2例并发症。结论应用子宫全切举宫器在腹腔镜下或辅助下的全子宫切术更方便安全、省时、出血少、并发症少。
Objective To evaluate the value of using the special uterovaginal elevator in total laparoscopic hysterectomy or laparoscopic assisted vaginal hysterectomy. Methods A retrespective analysis on 49 patients who underwent total laparoscopic hysterectomy or laparoscopic assisted hysterectomy was carried out , including operating time, bleed and complications loss. These cases were divided into two groups by different methods of operations. Group Ⅰ ： using the special utero - vaginal elevator in total laparoscopic hysterectomy . Group Ⅱ： using absorbent gauze to plug fornix and vaginal. Results The operating time in group Ⅰ （ 100.86 ± 25.04min） was signi/icantly shorter than that in group Ⅱ （ 116.5 ± 27.68rain）, p 〈 0. 05. The bleed in group Ⅰ （ 147. 24 ± 48.76ml） was significantly lower than group Ⅱ （179.5 ± 62.87ml）, p 〈 0.05. There was no complication in group Ⅰ and there were two complications in group Ⅱ. Conclusion By using the special utero - vaginal elevator in total laparoscopic hysterectomy or laparoscopic assisted vaginal hysterectomy, the operatians becomes more convenient and safer, with shorter operating time, lower bleed loss and fewer complications.
