摘要
目的总结骨科重大手术期间自体血回输的临床应用与护理。方法 20例骨科重大手术患者,于全麻后手术前行急性等溶血液稀释(ANH),采血200-400 ml,术中使用血液回收机回收术野出血,以洗涤红细胞的形式回输,术后除脊柱侧弯矫治术外,其余病例均回收手术创面引流血经过滤后回输。结果除3例脊柱侧弯矫治术术中输入库血外,其他病例均可做到不输库血。ANH期间血流动力学稳定,均未发生输血并发症。结论 ANH及术中自体血回收及术后引流血回收的综合应用,对患者凝血功能无明显的影响,能避免或减少输入库血。
Objective To study the nursing points and application of autotransfusion to major orthopetic operations. Methods 20 cases of major orthopetic operations received an autotransfusion regime of preoperative acute normovolemic hemodilution （ANH） and intraoperative blood by cell saver and postoperative wound drainage blood salvage. Results Patients except three cases of spine operations didn＇t need homologous blood transfusion. During ANH, the hemodynamics was stable and no complication of transfusion was found in all patients. Conclusion Autotransfusion regime effects the hemostasis function insignificantly and can effectively decrease the amount of homologous blood transfusion in major orthopetic operations.
