舒芬太尼复合异丙酚麻醉在ERCP中的应用 被引量:4

APPLICATION OF SURFENTANYL COMBINED WITH PROPOFOL FOR SEDATION IN ENDOSCOPIC RETROGRADE CHOLANGIOPANCREATOGRAPHY

摘要 目的观察静脉舒芬太尼复合异丙酚在逆行胰胆管造影(ERCP)中的麻醉效果及对呼吸循环的影响。方法68例ASAⅠ-Ⅱ级行ERCP的患者,随机分为两组,舒芬太尼复合异丙酚麻醉组(S+P组,n=34)和异丙酚麻醉组(P组,n=34)。观察两组异丙酚用量;术中呛咳、躁动情况;OAA/S评分;循环、呼吸变化和对操作记忆。结果麻醉诱导异丙酚用量和置镜时呛咳、躁动发生率P组大于S+P组(P〈0.05);血流动力学变化:P组麻醉后1min、3min血压较麻醉前下降(P〈0.05),5min后恢复至手术前水平;两组患者术中OAA/S评分、初醒时间无明显差异;对检查治疗过程无记忆。结论舒芬太尼0.1-0.15μg·kg^-1复合并丙酚静脉麻醉行ERCP安全、有效。 Objective To observe the sedative efficacy and the effect of surfentanyl combined with propofol intravenous infusion on respiration and circulation in Endoscopic Retrograde Cholangiopancreatography (ERCP). Methods Sixty-eight ASA grade Ⅰ-Ⅱpatients, undergoing ERCP, were randomly separated into two groups. Group Ⅰ ( n = 34) and group Ⅱ ( n = 34). The dosage of propofol, the rate of choke well and rash were recorded, and compared with OAA/S score, the changes of inspiration and circulation, the memory about operation in both groups, the all datum drawn from the study should be processed in statistic ways. Results The dosage of propofol, the rate of choke well and rash were significant higher in group Ⅱ than that in group Ⅰ during induced or inserting the endoscope; the MAP in group Ⅱ was decreased in 1 min, 3 min after sedation begun, compare with pre-sedation, there was significant difference, but it was recovery 5 minutes later. The OAA/S scores, the time of awake and the memory to operation had no significant difference in both groups. Conclusions It is safe and available for surfentanyl (0. 1 -0. 15 μg·kg^-1 )combined with propofol infusing intravenously to sedate in ERCP.
