肺表面活性物质与机械通气联用治疗新生儿肺透明膜病 被引量:1

TREATMENT OF PULMONARY HYALINE MEMBRANE IN NEWBORN WITH PULMONARY SURFACTANT AND MECHANICAL VENTILATION

摘要 目的探讨肺表面活性物质(PS)与机械通气联用治疗新生儿肺透明膜病(HMD)的疗效及临床价值。方法将71例HMD患儿随机分两组,治疗组36例早期足量给予PS并机械通气治疗,对照组35例常规机械通气治疗,两组均配以综合疗法,严格消毒管理。观察治疗前后血气、X线胸片及临床表现、并发症、住院时间及住院费用。结果两组患儿的血气较治疗前显著改善,两组比较无显著差异,而治疗组肺部感染、颅内出血等发生率明显低于对照组,氧疗时间、X线胸片恢复正常时间与住院时间明显短于对照组,而且住院费用也较低。结论PS与机械通气联用能迅速有效地治疗早产儿肺透明膜病,与常规机械通气比较,具有疗效好、并发症少、住院时间短及费用相对较少等特点。 Objective To explore the effect of pulmonary surfactant (PS) combined with mechanical ventilation on pulmonary hyaline membrane disease of newborn (HMD) in newborn. Methods 71 newborns with HMD were treated with mechanical ventilation and other conventional theories. Among them, 36 were in addition administered sufficient Curosurf (a kind of PS). All the patients were evaluated in blood-gas analysis, x-ray chest films, clinical symptoms, complications and time in hospital. Results The symptoms and blood-gas indices were obviously improved after treatment at different time from 0.5 h to 72 h in all patients, but the rates of lung infection and intracranial hemorrhage were obviously lower in Curosurf group than in only mechanical ventilation group. There had a shorter time in oxygen therapy and x-ray recovery in Curosurf group than in only ventilation group. The duration and the costs in hospital also decreased in Curosurf group. Conclusion Using PS combined with mechanical ventilation to treat HMD in newborn is more effective and cheap than only using mechanical ventilation.
