摘要
目的:观察前列腺素E1对周围神经损伤再生的促进作用,并与维生素B1,B6的疗效相比较。方法:收集北华大学附属医院手外科收治急诊前臂神经切割伤患者22例(30根神经:正中神经14根,尺神经16根),随机分为两组(n=11):①维生素B组:神经缝合术后口服维生素B1,B6各10mg/次,1次/d,连续用药3个月。②前列腺素E1组:神经缝合术后前列腺素E1150μg加入生理盐水500mL静点,1次/d,连续用药1个月。术后3个月开始每个月做肌电图检测,测定神经运动电位、感觉电位的潜伏期和波幅,统计最后一次检测值,两组进行比较。结果:术后平均随访7个月。22例全部进入结果分析。①至最后一次检测时维生素B组损伤神经中感觉电位恢复率28.6%,运动电位恢复率为85.7%;前列腺素E1组感觉电位恢复率为50%,运动电位恢复率为87.5%。②运动电位潜伏期:两组正中神经比较无显著性差异(P>0.05)鸦尺神经前列腺素E1组较维生素B组短5ms(P<0.01)。③运动电位波幅:正中神经:前列腺素E1组较维生素B组波幅高5mV(P<0.05),尺神经:前列腺素E1组较维生素B组波幅高8mV(P<0.01)。④前列腺素E1组尺神经感觉神经电位的潜伏期较维生素B组短4ms(P<0.01)。⑤两组比较尺神经的感觉神经电位的波幅差异无显著性意义(P>0.05)。结论:前列腺素E1能促进周围神经损伤后再生,其效果优于维生素B族。
AIM: To investigate the effect of prostaglandin E1 on peripheral nerve regeneration and compare it with that of vitamin B1 and B6. METHODS: A total of 22 cases of 30 acute nerve cut (14 median nerves and 16 ulnar nerves) at forearm level at Department of Hand Surgery, Affiliated Hospital of BeiHua University were studied. They were assigned randomly into 2 groups (n=11): ①Vitamin B group: The patients were given vitamin B1 and B6 taking orally after neurorrhaphy 10 mg every time, respectively, once a day for 3 months. ②Prostaglandin E1 group: 150μg proataglandin E1 was added into 500 mL saline after neurorrhaphy to perform dripped into vein, once a day for 1 month. Electromyography detection was conducted monthly from the 3^rd month after operation to measure neural motor potential, latency and amplitude of sensory potentiaL The last detective value was tested, and then to compare between the two groups. RESULTS: Follow up was 7 months after operation averagely, and totally 22 cases were involved in the result analysis. ①Till the last detection, the recovery rate of sensory potential in injured nerve in the vitamin B group was 28.6% and the recovery rate of motor potential was 85.7%. In the prostaglandin E1 group the recovery rate of sensory potential was 50% and the recovery rate of motor potential was 87.5%. ②Latency of motor potential: There was insignificant difference in the median nerve of the two groups (P 〉 0.05). The length of ulnar nerve in the prostaglandin E1 group was shorten about 5 ms than that of vitamin B group (P 〈 0.01 ). ③Amplitude of motor potential: Median nerve: The amplitude of wave in the prostaglandin E1 group was higher about 5 mV than that of vitamin B group (P 〈 0.05). Ulnar nerve: The amplitude of wave in the prostaglandin E1 group was higher about 8 mV than that of the vitamin B group (P 〈 0.01). ④The latency of sensory potential of the ulnar nerve in the prostaglandin E1 group was shorter about 4 ms than that of the vitamin B group (P 〈 0.01 ). ⑤There was insignificant difference of the amplitude of wave of sensory nerve potential of ulnar nerve between the two groups (P 〉 0.05). CONCLUSION: Prostaglandin E1, can promote peripheral nerve regeneration, and its effect is better than that of the vitamin B family.
