GSTM1空白基因型和吸烟与食管癌危险性的Meta分析

摘要 目的:对谷胱甘肽-S-转移酶M1基因空白基因型与吸烟在食管癌发生中的相互作用进行Meta分析。方法:按吸烟与否进行分层合并分析。以OR值作为每个研究结果的效应测定指标。若各研究结果之间无显著异质性则采用固定效应模型(FEM),否则使用随机效应模型(REM)进行合并,同时给出Meta分析森林图。基因-环境交互作用分析采用单纯病例分析法进行。结果:共收集国内外相关资料5篇,积累病例560例,对照743例,吸烟组合并OR为1.523(95%CI:1.099-2.109);不吸烟组合并OR为0.933(95%CI:0.469~1.855);单纯病例合并分析显示,GSTM1空白基因型与吸烟交互作用合并OR值为1.363(0.954~1.949)。结论:吸烟可能会增加GSTMl空白基因型个体患食管癌的危险性,即携带GSTM1空白基因型的吸烟者其患癌的危险性可能会增加,GSTM1空白基因型与吸烟之间在食管癌的发生方面可能存在一定的交互作用。 Objective: To evaluate the associations and interaction between glutathione S-transferase M1 (GSTM1) null genotype, smoking behaviour and the development of esophageal cancer by meta-analysis. Methods:Citations collected were stratified by smoking,and odds ratio was employed to evaluate the risk of esophageal cancer and GSTM1 status. To take into account the possibility of heterogeneity across the studies, a statistical test for heterogeneity was performed, and fixed or random effect meta-analysis model was used in terms of the result of heterogeneity test. And also the forest plots of recta-analysis were worked out. The gene-environment interaction of GSTM1 null genotype and smoking was assessed by case-only design. Results: A total of 560 cases and 743 controls from 5 studies Was included. In smoking groups, the pooled OR was 1. 523(1. 099-2. 109); in no smoking groups,the pooled OR was 0. 933(0. 469-1. 855). An association between GSTM1 null genotype and smoking was found among cases. The pooled OR of interaction was 1. 363(0. 954- 1.949). Conclusion:The smokers carting GSTM1 null genotype might be associated with the increased risk of esophageal cancer. There was a possible positive interaction between the effects of smoking and GSTM1 genotype on esophageal cancer risk.
