摘要
检测26例Ⅲ度烧伤病人伤后6d内血、尿肌红蛋白浓度的变化,结果发现病人尿肌红蛋白的动态变化与血相似,可通过测定尿肌红蛋白浓度来推测烧伤的严重程度;血肌红蛋白浓度与Ⅲ度烧伤面积呈正相关(r=0.8079,P<0.001);肌红蛋白血症的严重程度与病人早期死亡率无关,但可能与肾功能损害有一定关系。
Myoglobin (Mb)is the heme-containing protein involved in oxygen storage and transport in human skeletal and cardiac muscle. MB is released in to blood and cleared into urine by kidney due to deep burns or clcctric injurics,Myoglobulincmia and myoglobulinuria can often be observed. In some severe cases,the urine of the patients is brownish dark in eldor which may lead to acute renal failure.We have studied the changes of Mb concentration in blood and urine in 26cases with Ⅲ degree burn during 6 days postburn,It shows that dynamic change of patient's Mb concentration in urine and in blood is almost the same. From that we can infer the Mb concentr ation in blood by detecting Mb concentration in urine. Except the old group for its Mb concentration in urine is lower and dynamic change is smooth compared with the young one, while its Mb concentration in blood maintains high level. There is an interrelationship between Mb concentration in blood and Ⅲ degree burn surface area (r=0.8079,P<0.001); Myoglobinemia does nothing with mortality in early stage postburn (no patient died in our studied group);but it is possible that affectes the renal function.
