幽门螺杆菌感染对儿童身高生长的影响 被引量:8

Effect of helicobacter pylori infection on growth among asymptomatic children

摘要 目的观察幽门螺杆菌(Hpylori,Hp)感染对儿童身高生长的影响。方法福建医科大学附属第一医院儿科于2001年6月至2004年10月,采用整群抽样选择454例儿童进行血清却抗体检测、身高检查及社会经济状况和父母亲身高问卷调查。血清Hp抗体阳性者进行粪却抗原测定;血清Hp阴性者随访结束前再次检测血清Hp抗体。在研究开始、随访1年半及3年时分别进行1次体格生长指标测量及评价。结果60例(13.2%)儿童感染Hp,290例(63.9%)儿童未感染;Hp感染儿童3年身高增长值明显低于未感染儿童,调整后却感染和未感染儿童身高增长值及95%可信区间分别为21.4(20.6-22.3)cm、22.1(21.7-22.5)cm,两组校正均值比较,P=0.074。结论Hp感染与儿童体格生长有关联,主要体现在身高增长;Hp感染可能是影响儿童体格生长的环境因素之一,但影响的程度较弱;在社会经济状况相对不发达的地区,营养因素和遗传因素仍是儿童身高的主要影响因素。 Objective To observe the effect of helieobaeter pylori on growth of asymptomatie children. Methods In the First Affiliated Hospital of Fujian Medical University,454 children in general good health, aged 2 to 7 years, who surveyed for the socioeconomic status, parental height and weight and serum H. pylori IgG antibody,were followed up for growth of height for 3 years from Jun. 2001 to Oct. 2004. Anthropometrie measurements were performed every 1.5 years. Stool H. pylori antigen was measured with ELISA among participants who were H. pylori seropositive and serum H. pylori IgG antibody was measured again in the H. pylori seronegative participants before the follow-up survey was over. H. pylori status was defined as positive when both serum and stool antigen tests were positive, and was defined as negative when serum antibody test was negative both before survey and after follow-up survey. Centile values and Z score for height ( HAZ ) were calculated by using standard height and weight charts of WHO/NCHS. The third eentile value for height as the threshold values for stunting. Compare the differences of the height, HAZ and their increase between H. pylori infected and non-infected children. Multivariate eovarianee analysis were performed to evaluate the different level of the height increase. All the data were managed by EPI Info 5.01a and analyzed by SPSS( version 10.0 ). Results Totally 60 children were diagnosed as H. pylori positive and 290 as H. pylori negative. The increase of height in H. pylori infected and non-infected children were 21.1 ± 2. 9 cm and 22.2 ± 3. 3 cm ( P = 0. 011 ) by Mann-Whitney test ,and were 21.4 (20. 6 ± 22.3 )cm and 22. 1 ( 21.7 ± 22.5 } )cm ( P = 0. 074 ) by analysis of eovarianee after adjusting for possible confounding factors ,respeetively. The difference of height and HAZ standardized score were not statistically significant ( P 〉 0. 05 ) between H. pylori infection and non-infection individuals. Conclusion H. pylori has effect on growth of children, which is mainly manifested in the increase of height. Impact of H. pylori on growth of children is chronic and latent,and the magnitude of its impact is small.
