摘要
目的:总结应用腹腔镜手术联合十二指肠镜、胆道镜对胆囊结石合并胆管结石患者的治疗经验。方法:2002年3月至2005年6月,针对118例胆石病患者,43例应用腹腔镜联合十二指肠镜;75例采用胆道镜配合外科手术进行治疗。结果:118例手术均获成功,43例应用腹腔镜联合十二指肠镜患者中28例先行腹腔镜胆囊切除术(LC),4例术后急诊行十二指肠镜治疗,24例择期行十二指肠镜治疗。75例外科手术配合胆道镜取石,胆道结石一次性取净率达72%(54例),其余21例有16例经T管道取净,使胆道结石总取净率达93.3%(70/75)。结论:腹腔镜联合十二指肠镜治疗胆囊结石并胆管结石是有效的首选微创手术。
Objective.Summary application laparoscopy union duodenoscopy, biliary duet mirror coordination surgical operation to cholecystolithiasis merge bile duct stone patients treatment experience. Methods：In view of 118 examples cholelithiases patient,43 examples applications laparoscopy union duodenoscopy ;75 examples use the choledochoscopy coordination surgical operation to carry on the treatment. Results： 118 examples surgeries attain successfully,43 examples laparoscopy union duodenoscopy use, 28 examples advance LC surgery. Alter 4 examples techniques emergency medical treatment good duodenum mirror treatment,24 examples pick an auspicious date the good duodenoscopy treatment. 75 examples surgical operation coordinates the choledochoscopy to take the stone. The biliary duct stone disposable takes only rate reaches 72% （54examples）. Other 21 examples have 16 examples to take after the ＂T＂ tube tunnel only. Causes the biliary duct stone always to take only rate reaches 93.3% （70/75）. Conclusions：The laparoscopy union duodenoscopy treatment cholecystolithiasis and the bile duct stone are the effective first choice micro creates the surgery method. But its surgery indication relative is narrow.
