摘要
通过36例慢性上颌窦病变经鼻内窥镜手术与常规Caldwell-Luc氏手术对照分析发现:对慢性上颌窦炎性病变采用鼻内窥镜手术处理符合生理,操作简便,病人痛苦小,效果好,是对慢性上颌窦炎治疗的首选方案。但对上颌窦囊肿,特别是位于窦内下角的囊肿,经鼻内窥镜手术,往往由于术中出血病灶显示不清而切除不彻底,容易复发,不如Caldwell-Luc氏手术效果好。对上颌窦内病变的探查定性,鼻内窥镜下手术足以完成,无需常规Caldwell-Luc氏手术。
Thirty-six cases of chronic maxillary sinus disease treated by endoscopic endonasal surgery or Caldwell-Luc's operation were compared and studied. It was found that treating chronic infections of maxillary sinus by endoscopic endonasal surgery accorded with the physiology) the operation was sim-ple; pain was small and results were better. Therefore the endoscopic surgery was the first choice-Treating the cyst of maxillary sinus (particularly at the interior and inferior corner of the maxillary sinus )by the endoscopic surgery , relapse of the cyst occurred frequently, because the total resection of the cyst was difficult to perform by endoscopic surgery. Therefore, patients with sinus cyst should be treated by Caldwell-Luc's operation. But this conventional operation was. not always needed to treat other chronic maxillary sinus diseases,as the endoscopic surgery was more convenient and less painful.
