摘要
目的采用实时三维超声心动图(RT-3DE)探讨陈旧性心肌梗死患者二尖瓣环的几何形态变化。方法对21例心肌梗死患者进行RT-3DE成像,并与12名正常人进行对照,分析其二尖瓣环的几何形态变化。结果心肌梗死患者中,伴有显著二尖瓣反流患者的二尖瓣环显著扩张;二尖瓣环较正常对照组瓣环平坦;二尖瓣环在心动周期中的面积变化率降低。不伴有显著二尖瓣反流患者的二尖瓣环除瓣环高度显著小于正常组外,其瓣环面积、心动周期中面积变化率、周长以及瓣环直径与正常组之间无显著差别。结论二尖瓣环的大小和形态变化是发生缺血性二尖瓣反流的重要因素。
Objective Mitral annular geometry was evaluated by real-time three-dimensional echocardiography (RT-3DE) in patients with old myocardial infarction (OMI). Methods Twenty-one patients with myocardial infarction were imaged by RT-3DE. Mitral annular geometry change was analyzed and compared with that of 12 normal persons. Results In patients with OMI, those patients with significant mitral regurgitation (MR), mitral annulus dilated significantly and was more flatter than normal control group. Mitral annular area change rate from maximum to minimum during the cardiac cycle was lower (17 % ± % vs 30 % ± 5 %, P〈0.0S). There was no significant difference in patients without significant MR compared with normal control group except their mitral annular height was shorter than normal control group. Conclusion Changes in mitral annular size and shape markly contributed to ischemic MR.
