摘要
目的探讨微乔网包裹脾脏治疗外伤性脾破裂的可行性及临床意义。方法回顾性分析我院1999年10月～2005年09月采用微乔网行脾脏包裹术治疗外伤性脾破裂21例,BuntainCT分级I型4例,Ⅱ型10例,Ⅲ型7例,有其它器官合并伤8例。结果21例均顺利完成手术,手术时间50～150min,平均86min。2例术后出现大出血,再次手术行脾切除。术后恢复正常工作时间:13例单纯脾破裂20～30d,有合并伤8例30～100d。结论用微乔网包裹脾脏治疗外伤性脾破裂切实可行,适用于BuntainCT分级Ⅰ～Ⅲ型脾脏损伤且无严重合并伤的患者。
Objective To explore the feasibility and significance of the Method with Vicryl mesh for treating traumatic ruptured spleen.Methods From October 1999 to September 2005,21 cases with traumatic rupture of spleen received Vicryl mesh procedure were analyzed retrospectively. According to the Buntain classification of CT-graded splenic injury,4 cases were classified as ＂type Ⅰ ＂, 10 cases as ＂type Ⅱ＂ and 7 cases as ＂type Ⅲ＂. Combined other organ injuries had 8 cases. Results The operation was completed sueeessfuUy in 21 cases. The operative time was 50-150 min （mean,86 min）. 2 cases were transferred to perform re-operation of splenectomy because of postoperative massive bleeding. Postoperative time to normal activities was 20-30 days in 13 cases of simple ruptured spleen and 30-100 days in 8 cases of ruptured spleen combined with other injuries. Conclusions Vicryl mesh procedure is a feasible mathod in the treatment of traumatic rupture of spleen.It is suitable for ＂type Ⅰ ,Ⅱ orⅢ ruptured spleen and without severe combined injuries.
