摘要
目的:分析慢性中重度牙周炎患者局部应用盐酸米诺四环素(MINO)后,不同牙周状态下龈沟液(GCF)中IL-8总量和浓度的差异及其与临床指标的关系;评价局部应用MINO在牙周病药物治疗中的疗效。方法:采用随机单盲法抽取用药组与对照组38例。用药组将盐酸米诺四环素软膏置于牙周袋内作为局部药物治疗;以单纯基础治疗为对照组。分为用药组27颗患牙,108个位点;对照组11颗患牙,44个位点。于基线前两周完成全口龈上洁治及口腔卫生宣教。基线时收集相应牙位龈沟液(GCF)样本,用ELISA法检测各样本中IL-8的总量和浓度,并观察基线时龈沟出血指数(SBI)、牙周探诊深度(PD)、牙周附着水平(AL);然后行龈下刮治术,用药组龈沟内放药,对照侧不放药。第2周牙周指标检查同上。结果:用药组龈沟液中IL-8浓度和总量水平与其基线和对照组相比明显下降(P<0.05),其临床指标用药组与对照组相比改善明显(P<0.05)。结论:盐酸米诺四环素软膏(MI-NO)辅助治疗牙周炎,能有效降低龈沟液中IL-8的总量和浓度,改善牙周组织状况。
Objective: To analyze the changes of interleukin-8 levels in gingival crevicular fluid(GCF) and their relationships with clinical parameters after subgingivally administering 2% minocycline-HC1 ointment to the periodontal pocket as the auxiliary treatment of the moderate and severe chronic periodontitis. Methods. Thirty-eight subjects were collected and divided into two groups with a randomized single blind method. Minocycline-HCI ointment was administered into periodontal pockets as the treatment group including 108 sites, 27 teeth and the control group with only single initial therapy including 44 sites, 11 teeth. Supragingival scaling and oral hygiene instruction were finished two weeks before baseline. In baseline and two weeks later, gingival crevicular fluid samples were collected and detected with ELISA technique, and probing depth (PD), attachment level (AL) and sulcus bleeding index (SBI) were observed. Results. The Interleukin8 amounts and concentration were reduced (P〈0.05) in the treated group than those of the baseline and the control group. Parameters in treated group were significantly improved than those of control group (P〈0.05). Conclusion. Administration of the local drug delivery system of 2% minocycline-HCI ointment as the auxiliary treatment of moderate and severe chronic periodontitis could reduce the levels of interleukin-8 in GCF and improve the periodontal tissue conditions.
