肾上腺意外瘤中亚临床库欣患者代谢综合征表现 被引量:6

Metabolic Syndrome in Subclinical Cushing’s Syndrome of Adrenal Incidentaloma

摘要 目的评价肾上腺意外瘤中亚临床库欣患者代谢综合征的发生情况。方法检测19例亚临床库欣患者的血压、血脂、血尿皮质醇、促肾上腺皮质激素(ACTH)等指标,并行口服糖耐量试验,检测结果与20例肾上腺腺瘤型的显性库欣患者和20例正常对照者比较。结果与性别、年龄和体质量指数匹配的对照组比较,亚临床库欣组患者的胰岛素敏感指数、HOMA指数、甘油三酯、胆固醇和收缩压均升高明显,其糖尿病、糖耐量异常、高血脂和高血压的发生率均明显高于对照组;而上述指标在亚临床库欣组与同为肾上腺腺瘤型的显性库欣组间比较,均无统计学差异。血尿皮质醇三组中以显性库欣组最高,亚临床库欣组次之,对照组最低,组间比较均有统计学差异。结论亚临床库欣患者尽管皮质醇分泌轻度升高,尚不足以发展成显性库欣综合征,但已经使糖尿病、高血脂、高血压等代谢综合征症状的发生率升高,提示亚临床库欣的存在对患者造成一定的危害性,且危害程度与显性库欣综合征相仿。 Objective To explore some features of metabolic syndrome in patients with subclinical Cushing's syndrome (SCS) of adrenal incidentaloma. Methods Nineteen patients of SCS were evaluated. All of them took measurement of blood pressure, triglyceride (TG), total cholesterol (TC) , HDL cholesterol, plasma cortisol, urina- ry free cortisol, ACTH and underwent oral glucose tolerance test (OGTT). The results were compared between 20 patients of overt Cushing's syndrome with adrenal adenoma and 20 controls. Results In comparison to the controls matched for sex, age, and BMI, the patients of SCS had higher prevalence of diabetes and IGT, as well as higher insulin sensitivity index (ISI) and HOMA index. The prevalence of hyperlipidemia together with TG and TC was higher. The prevalence of hypertension with systolic blood pressure was also higher than the controls. There was significant difference in plasma and urinary cortisol in all three groups; the overt Cushing's syndrome group was the highest while the control group was the lowest. Conclusion Although the amount of cortisol excess was insufficient to give a clear Cushing phenotype, it might induce some features of the metabolic syndrome, in diabetes mellitus, hypertension, and hyperlipidemia. SCS may do harm to the patients just as overt Cushing' s syndrome.
