摘要
目的:探讨超声消融术在治疗血管闭塞性疾病方面的研究应用成果及其发展趋势。资料来源:应用计算机检索Medline1980-01/2004-01期间与超声消融术治疗血管闭塞性疾病相关的文献,检索词:“ultrasonicbioffects,ultra-sonicangioplasty”,并限定语言种类为“English”;同时检索清华同方数据库1994-01/2005-01期间的相关文献,检索词:“超声消融、体外治疗性超声、血栓、动脉硬化”,并限定语言种类为中文。资料选择:从资料中选择有关超声治疗血管闭塞性疾病的基础和临床研究方面的文献,观察对象无论是患者还是动物均纳入,排除综述类文献。资料提炼:从筛选到的120篇文献中,选择研究设计较严密、临床研究样本较大的文献,重复的资料以近年发表在较权威杂志者优先,共纳入21篇文献进行综述。资料综合:21篇文献中包括14篇体外实验,8项临床研究共248例患者,分别就超声消融治疗的安全性、疗效、治疗方法以及和其他治疗方法对照或联合运用的效果进行评价。结论:实验室研究证实了超声消融治疗的安全性和有效性,超声血管成形术的临床运用显示了它较其他介入治疗所特有的优势,体外治疗性超声的临床应用前景广阔。
OBJECTIVE： To probe into the application of ultrasonic thrombus ablation in the treatment of vaso-occlusive diseases and the tendency of its development.
DATA SOURCES： We searched Medline for relevant documents on ultrasonic thrombus ablation in the treatment of vaso-occlusive diseases published between January 1980 and January 2004 with the key words of ＂ultrasonic bioeffects＇and ＂ultrasonic angioplasty＂ in English. At the same time, we also searched Tsingghua Tongfang d＊atabase for relevant documents published between January 1994 and January 2005 with the key words of ＂ultrasonic melting,extracorporeal treatment by ultrasonic method, thrombus, arteriosclerosis＂ in Chinese.
STUDY SELECTION： The documents relating to the basis and clinic application of treating vaso-occlusive diseases in an ultrasonic way were selected. Articles on no matter observational subjects were patients or animals were included. Reviews were excluded.
DATA EXTRACTION： From the selected 120 documents by means of screening method, 21 documents published in more authoritative journals in recent years and with more strictness in research and design and more samples in clinical research were drawn out for reviews.
DATA SYNTHESIS： Among the 21 documents, there are 14 documents on experiment in vitro, including 8 items of clinical researches with a total of 248 patients, making evaluation for the s^ety, curative effect and therapeutic methods of ultrasonic treatment as well as the comparison with other treatments or the effect of combined application, respectively.
CONCLUSION： Laboratory study verifies the safety and effect of ultrasonic treatment. The clinical application of ultrasonic angioplasty shows its special superiority as compared with other interventional treatments and the wide prospect of ultrasonic clinical application in treatment in vitro.
