摘要
目的:通过对中耳手术相关的解剖结构进行观察、测量,为中耳手术提供理论参考。方法:实验于2005-05/07在珠江医院耳鼻咽喉科实验室进行。对10个尸头20侧颞骨标本进行解剖,耳乳突解剖模仿闭式乳突根治术、面神经减压术和鼓室成型术、经典乳突根治术等手术步骤进行。在手术显微镜下观察、测量中耳手术的相关解剖数据。结果:水平半规管与面神经之间的最短距离为(1.587±0.206)mm,后半规管与面神经垂直段之间的最短距离为(2.514±0.386)mm,水平半规管与砧骨短脚之间的最短距离为(2.422±0.251)mm,面神经垂直段与鼓沟在圆窗平面的距离为(3.622±0.369)mm,面神经垂直段与鼓沟在锥隆起平面的距离为(3.562±0.217)mm,面神经垂直段与圆窗龛的距离为(4.830±1.152)mm,面神经与鼓索神经在圆窗平面的距离为(2.480±0.274)mm,面神经水平段与镫骨底板的最短距离为(2.248±0.204)mm。结论:相关测量数据为中耳手术中避免损伤面神经等重要结构提供了参考。
AIM: To provide theoretical reference for operation on the middle ear through the observation and measurement of related anatomic structure of operation on the middle ear.
METHODS: The experiment was conducted at the laboratory of Department of Otorhinolaryngology, Zhujiang Hospital between May and July 2005. Twenty human temporal bones of 10 dead bodies were dissected. Dissection of ear mastoids borrowed the procedures of closed radical mastoidectomy, facial nerve decompression, tympanoplasty and typical radical mastoidectomy, etc. Relative anatomical data of operation on the middle ear were observed and measured under operating microscope.
RESULTS: The shortest distance between lateral semicircular canal and facial nerve was (1.587±0.206)mm. The shortest distance of the vertical portion between posterior semicircular canal and facial nerve was (2.514 ± 0.386) mm. The shortest distance between lateral semicircular canal and anvil short crus was (2.422±0.251) mm. Distance between the vertical segment of facial nerve and tympanic ditch was (3.622±0.369) mm at the position of round window. Interspace between the vertical portion of facial nerve and tympanic ditch was (3.562±0.217) mm at the position of pyramidal eminence. Distance between vertical segment of facial nerve and cochlear window was (4.830±1.152) mm. Distance between facial nerve and chorda tympani nerve at level of round window was (2.480±0.274) ram. Stapedial board was (2.248±0.204) mm underneath to the horizontal portion of facial nerve.
CONCLUSION: These anatomical parameters provide a reference value to avoid the trauma of facial nerve when do surgery in the middle ear.
