Influence of substance P on the release of histamine by mast cells in hypertrophic scar and its significance

摘要 目的探讨神经肽P物质(SP)与增生性瘢痕瘙痒发生的关系及可能的机制。方法用透射电镜观察人增生性瘢痕、非增生性瘢痕以及皮肤中肥大细胞(MC)超微结构特点及其功能状态;I型胶原酶和透明质酸酶消化分离肥大细胞,用荧光分光光度法检测SP对分离的MC组织胺释放率的影响,观察时间及剂量-效应关系。结果(1)增生性瘢痕中MC占有核细胞的百分比(8.6±1.9)%显著高于非增生性瘢痕(6.1±1.5)%及皮肤组织[(5.3±1.2)%,P<0.01]。(2)多数增生性瘢痕组织中的肥大细胞呈活化状态。(3)增生性瘢痕组织中MC的组织胺释放率随SP刺激时间的延长和浓度的增加而递增,与非增生性瘢痕组及皮肤组差异有统计学意义(P<0.01)。结论SP明显提高增生性瘢痕中MC释放组织胺的水平而产生痒觉,这可能是增生性瘢痕瘙痒发生的机制之一。 Objective To study the relationship between substance P and the itching of hypertrophic scar. Methods The ultrastructural characteristics and functional status of mast cells (MCs) in hypertrophic scar, non-hypertrophic scar and normal skin were observed by transmission electron microscopy. MCs were separated by collagenase and hyaluronidase and fluorospectro-photometry was employed to examine histamine after inducement of SP at different concentrations and time points in the MCs. Results The ratio of MCs in hypertrophic scar (8, 6 ± 1,9) was significantly higher than that in non-hypertrophic scar (6.1 ± 1.5) and normal skin (5.3± 1.2, P〈0.01). SP enhanced histamine release of MCs in hypertrophic scar dose-dependently and time-dependently, which showed significant difference as compared with non-hypertrophic scar group and normal skin group (P〈 0.01 ). Conclusion The itching complaint occurred with the significantly higher histamine level in hypertrophic scar stimulated by SP. This effect may be one of mechanisms of pruritus in hypertrophic scar patients.
