摘要
目的利用隐动脉膝上皮支筋膜皮瓣修复腘窝部瘢痕挛缩。方法根据解剖学的研究,利用隐动脉在膝上发出的筋膜皮支为血管蒂,以收肌结节上1.5m为旋转轴点,设计隐动脉膝上皮支筋膜皮瓣,进行腘窝部瘢痕挛缩的修复。结果临床治疗腘窝部瘢痕挛缩21例,皮瓣的长度为14~20cm,宽度为8~10cm,其中有3例出现皮瓣远端坏死,余成活良好。结论隐动脉膝上皮支筋膜皮瓣修复腘窝部瘢痕挛缩,取得了修复区和供皮区一期修复的良好效果,并保证了术后修复区的良好功能,是一种比较理想的手术方法。
Objective To repair the scar contracture of popliteal fossa with retrograde fasciocutaneous flap with the cutaneous branch above the knee of the saphenous artery. Methods Based on the study of anatomy, the retrograde fasciocutaneous flap with a pedicle of superagenual cutaneous artery was designed to repair scar contracture of popliteal fossa. The rotation axis point was 1.5 cm proximal the adductor tubercle. Results Twenty-one patients with scar contracture of popliteal fossa were treated using this flap clinically, the length of the which was from 14cm to 20cm and the width was from 8cm to 10cm. Only 3 cases partially necrosed at distal partion, the other survived completely. Conclusion Applying retrograde fasciocutaneous flap with a pedicle of superagenual cutaneous artery to repair scar contracture of popliteal fossa is a good operation method with the advantages of good results of primary repair of both the supplied region and the accepted region.
