摘要 目的研究手臂振动病的X射线及磁共振成像(MR/)表现,重点研究其早期诊断价值。方法所有142例病人全部进行X射线和MR/检查,按照工龄分为5组,分别统计X射线及MRI征象,对各种征象进行X射线和MR/比较分析;MR/与对照组比较分析;各工龄组问比较分析。结果X射线显示征象以骨质疏松、骨质硬化、腕骨坏死为主;MRI除显示上述征象外,还能够显示关节积液、骨髓水肿、软组织损伤等。以井下非振动作业工人为对照组进行比较,骨髓水肿和腕骨坏死两项在对照组中未显示;关节积液、骨质疏松、骨质硬化3项MRI显示数量明显高于对照组,两组比较差异有非常显著性(x^2=130.82,10.42和10.39,P〈0.01);各工龄组问比较,MRI征象发生高峰在1-5年组;X射线征象发生高峰在21年以上组。结论MR/可以早期显示手臂振动病的骨关节改变,明显早于X射线;MR/所显示的征象多数与振动作业有密切相关性。 Objective To research the magnetic resonance imaging (MR]) and X-ray features of hand-arm vibration disease, especially to research their values for early diagnosis. Methods 142 cases were examined with X-ray and MRI. All cases were divided into 5 groups based on their length of service. X-ray and MRI findings were compared and analyzed to the controis and between the groups of different length of service. Results Osteoporosis, osteopathia condensans and avascular necrosis of ossa carpi were the main findings in X-ray. In addition to aforementioned features, MRI also showed hydrathrosis, osseous e- dema and parenchyma damage. The controls, who worked underground but without vibration, there were not findings of osseous edema and avascular necrosis of ossa carpi. The findings of hydrathrosis, osteoporosis and osteopathia condensans were much more obvious than those found in the control group. There was a significant difference in MRI between the two groups( X2 = 130. 82,10.42,10.39, P 〈 0.01 ). Compared by the length of service, many abnormal MRI signs were seen in the 1-5 years group, while X-ray were in the 〉21 years group. Conclusion MRI can display bone and joint diseases of vibration much earlier than X-ray does. A lot of signs of MRI are well correlated with vibration work.
