摘要
目的探讨超声引导下介入治疗并注入不同药物治疗卵巢内膜异位囊肿、卵巢单纯性囊肿和盆腔包裹性积液的疗效。方法经阴道超声引导下对74例盆腔囊肿进行78次穿刺,失访10例,比较64例穿刺疗效。卵巢内膜异位囊肿穿刺后注入孕激素或无水乙醇+孕激素;卵巢单纯性囊肿注入无水乙醇或无水乙醇+头孢噻肟钠;包裹性积液注入糜蛋白酶或头孢噻肟钠。穿刺前均行血清肿瘤标记物CA125、癌胚抗原(CEA)、甲胎蛋白(AFP)检测,结果基本正常。结果随访1~3个月,卵巢内膜异位囊肿28例,穿刺有效率为89.6%,注射无水乙醇+孕激素亚组术后1个月的囊肿平均容积为原囊肿平均容积的19.9%,显著高于单用孕激素亚组的30.4%(P<0.05)。卵巢单纯性囊肿25例28次穿刺,有效25次,有效率为89.3%。包裹性积液11例12次穿刺,有效率为81.8%,2例近期复发。结论经阴道超声引导下穿刺治疗盆腔囊肿为一种安全、便捷、经济、有效的方法,值得推广。
Objective To appraise the therapeutic effects of ultrasound-guided aspiration plus different drugs in endometriotic cysts,simple cysts and encapsulated pelvic effusion. Methods Aspiration of pelvic cysts was carried out transvaginally in 74 patients, ten of whom were lost to be follow-up,hence omparsion of the effectiveness was confined to 64 patients only. Following the aspiration, progestational hormone or progestational hormone plus ethanol was injected into the endometriotic cysts ; ethanol or ethanol plus cefotaxime sodium was injected into the simple cysts; chymotrypsin and eefotaxime sodium were injected into the encapsulated pelvic effusion, Before aspiration , all patients were phlehotomized to make sure the tumor markers CA125, CEA, AFP were negative. Results All those cases were followed up for one to three months, The effective rate in 28 patients with en dometriotic cysts was 89.6%. After one month, the mean cystic volume was reduced to 19.9% of their original volume in those given progestational hormone and ethanol, while those only given progestational hormone reduced to 30. 4%; 28 aspirations were performed in 25 patients with simple cysts and the curative rate was 89.3%. 12 aspirations were done in llpatients with encapsulated pelvic effusion and the curative rate was 81. 8%. Short term recurrence occurred in two patients. Conclusions The ultrasound-guided aspiration of pelvic cysts is simple, effective, safe, and less expensive which worths to he recommended.
