摘要
为研究氧、异搏停、酚妥拉明,硝酸甘油、硫氨卓酮、硝苯砒啶和巯甲丙脯酸在2260m高原对肺动脉压的影响,对218例心血管病人进行了305例次的上述药物的经心导管监测的肺动脉压降压效果观察。结果:吸100%氧10分钟对不同水平肺动脉压均显示有统计学意义的降压效应,下降幅度为0.17~0.7kPa。肺动脉内注射异搏停10mg,肺动脉内注射酚妥拉明10mg,舌下含化硝酸甘油1mg和口服琉甲丙脯酸75mg只对肺动脉压增高者显示有统计学意义的降压效应,异搏停下降幅度为0.36~0.5kPa,酚妥拉明下降0.63~0.85kPa,硝酸甘油下降0.46~1.21kPa,巯甲丙脯酸下降0.56~0.94kPa。异搏停、酚妥拉明、硝酸甘油和巯甲丙脯酸对肺动脉压正常者虽显示降压作用,但无统计学意义。口服硫氮卓酮120mg和口服硝苯吡啶30mg对不同水平肺动脉压均未显示有统计学意义的降压效应。结论:吸氧、静注酚妥拉明、舌下含化硝酸甘油或口服巯甲丙脯酸可在2260m高原较明显地降低肺动脉压。
o study the effects of oxygen,verapemil pulmonar arterial ressure(Ppa)at 2260m altitude.218 cases underwent 305 times observations on the effects of the above mentioned drugs on Ppaunder catheterization monitoring.100%oxygen inhalation(10min,)showed significant decrease ofPpa,decreased rangc 0.17~0.7kPa,verapamil 10mg or phentolaminc 10mg infected slowly intopulmonary artery,sublingual nitroglycerin lmg or eral captopril 75mg only showed Ppa decreasein the patients with pulmonry hypertension. The decreased range of Ppa of verapamil was 0.36~0.5kPa,phentolamine 0. 63~0.85kPa,nitroglycerin 0.46~1.21kPa captopril 0.56~0.94kPa.Verapamil,phentolatnine,nitroglycerin and captopril showed decrease of Ppa in Ppa-normal pa-tients,but without statistic significance. Oral diltiazem 120mg or orai nifedipine 30mg showed nosignificant Ppa decreasc.Oxygen inhalation,intravenous phentolamine,sublingual nitroglycerin ororal captopril decreased pulmonary arterial presstire at 2 260m altitude.
