NICU中早产儿营养状况分析 被引量:3

摘要 【目的】对NICU中孕周小于34周的早产儿的营养摄入及体重增长状况进行分析,以期对临床上早产儿的营养支持策略提供依据。【方法】对生后24 h内入陕西省妇幼保健院NICU孕周小于34周、无消化系统畸形的早产儿64名,按出生体重分为体重≤1 500 g组和体重>1 500 g组,计算生后第1、2、3周热量和蛋白质的摄入、住院天数、开奶日龄、足量喂养日龄、体重开始增长日龄、回到出生体重日龄、静脉营养时间以及并发症的发生率。【结果】体重≤1 500 g组热量和蛋白质的摄入低于体重>1 500 g组,在第2周差异有显著性(P<0.05)。体重≤1 500 g组由于并发症发生率高,在开奶日龄、体重开始增长日龄、回到出生体重日龄及静脉营养时间均明显延迟,两组比较差异有显著性(P<0.05)。64例早产儿在出院时小于胎龄儿发生率高达68.75%。【结论】孕周小于34周的早产儿在出生后宫外发育迟缓发生率较高。应积极治疗和预防并发症,给予低热量静脉营养,提倡微量喂养,出院后有必要续贯治疗,以实现追赶性生长。
