摘要
目的在第2足趾移植再造拇指术中,应用短伸肌腱修复供足第1跖骨深横韧带,以减少术后供足并发症。方法在55例拇指再造术中自体55例健康足(55足)为术前对照组(A组);19例(19足)为本手术组(B组),36例(36足)为常规手术组(C组)。术后随访1a,应用单足足跟抬起站立试验与抬起次试验评价供足功能,并进行统计学分析。结果单足足跟抬起站立试验B、C两组比较P<0.05(χ2检验),单足足跟抬起次数试验B、C两组比较P<0.05(q检验)。结论应用短伸肌腱修复第1跖骨深横韧带能够改善术后供足功能、减少术后并发症。短伸肌腱修复供足第1跖骨深横韧带是一种简单、快速、不增加手术难度和损伤的方法。
Objective: In the operation of thumb reconstruction utilizing the second toe, repaired the first deep transverse metatarsal ligament using tendon extensor hallucisf brevis to reduce postoperative complications of providing foot. Methods: 55 cases were divided into group A,B and C. Group B(19 case) were repaired the first deep transverse metatarsal ligament with tendon extensor hallucisf brevis. Group C were carried out routine operation. All ease were compared with each other (group A) and analyzed statistically in terms of function. Results: In the test of standing on single foot with heel off, there was s significant difference between group B and group C ( P 〈0. 05, χ^2test). In the test of raising single foot heel continously, group B compared with group C, P 〈0.05( q test) Conclusions: Repairing the first deep transverse metatarsal ligament by using tendon extensor hallucis brevis could reduce the degree of anterior foot fallen and complications, and improve the function of anterior foot. Utilizing tendon extensor hallucis brevis to repair the first deep transverse metatarsal ligament is a simple, quick and effective operation that did not add operative difficulty and damage.
