鲑鱼降钙素、α维生素D_3与碳酸钙对老年脑卒中患者骨钙代谢的影响 被引量:14

摘要 目的评价联合应用鲑鱼降钙素、维生素D3与碳酸钙对老年脑卒中后肢体瘫痪患者骨、钙代谢的影响。方法老年脑卒中患者共364例,随机分为治疗组与对照组。治疗组予鲑鱼降钙素50IU,肌注,1次/周;α维生素D30.25μg,口服,1次/d;碳酸钙0.15g,口服,3次/d;另予抗血小板、神经营养药物、活血化瘀冶疗等,疗程1年。对照组不予鲑鱼降钙素、α维生素D3与碳酸钙,其余治疗同治疗组,疗程1年。治疗前后测定血Ca、碱性磷酸酶(ALP)、骨钙素(BGP)、甲状旁腺激素(PTH)、尿轻脯氨酸(HOP)、尿Cr。结果两组治疗前各生化指标无统计学差异(P均〉0.05),治疗组的血ALP、BGP及尿HOP/Cr在1年后显著下降(P均〈0.001),血Ca、PTH在治疗前后无明显变化(P均〉0.05)。对照组脑卒中1年后血ALP、BGP、Ca、PTH及尿HOP/Cr明显升高(P〈0.01~0.001)。两组治疗后各生化指标均有统计学差异(P均〈0.001)。结论联合应用鲑鱼降钙素、α维生素D3与碳酸钙能明显改善老年脑卒中患者肢体瘫痪后骨、钙代谢异常。 Objective To evaluate the effects of salmon calcitonin,α - hydroxyvitamin D3 and calcium supplementation on bone and calcium metabolism in disabledclderly stroke patients.Methods The study was conducted on 364 patients with hemiplegia after stroke.who were aged at least 60 years.The patients were randomly divided into two groups: (1) patients received salmon calcitonin, 1α- hydroxyvitamin D3, and calcium supplementation for 1 year. (2) patients were not given salmon calcitonin, 1α hydroxyvitamin D3 and calcium supplementation. Blood Ca, ALP, BGP, PTH, urinary HOP and urinary Cr were measured before and after treatment. Results Over a 1 - year follow - up interval, the levels of blood ALP, BGP and urinary HOP/Cr decreased significantly ( P 〈 0.001 ) in cases. No significient changes were found in the levels of blood Ca and PTH incases( P 〉 0.05) .Over a 1 - year follow - up interval,The level of blood ALP,BGP, Ca, PTH, urinary HOP/Cr in creased significantly(P 〈 0.01 - 0.001) in controls. Conclusion Treatment with salmon calcitonin, α- hydroxyvitamin D3 , and calcium supplementation can improve bone and calcium metabolism in disabled elderly stroke patients.
