冠状动脉瘘的彩色多普勒超声诊断分析 被引量:2

摘要 目的通过分析冠状动脉瘘的血流动力学及彩色多普勒心动图像特征的规律性,探讨其诊断冠状动脉瘘的准确性。方法总结1997年至2005年间8例冠状动脉瘘患者(均经手术证实)及近年来核心期刊报道的145例共153例冠状动脉瘘的彩色多普勒二维超声心动图特征,对其进行荟萃分析,寻找冠状动脉瘘的血流动力学及图像特征的规律性。结果根据图像特征,彩超对133例可作出正确诊断,诊断准确率87%。其二维及彩色多普勒血流图及频谱特征明显,可显示受累冠状动脉开口、内径、追踪其走行至瘘口破入哪个房室腔,对手术及介入治疗均具有指导作用。结论彩色多普勒二维超声心动图对冠状动脉瘘具有确诊价值。 Objective Through analysis of coronary artery fistula to find rules of dynamic and characteristic of color blood Doppler two-dimenstonal ultrasonnic echocardiogram, to find the accuracy of diagnosis of coronary artery fistula. Methods From 1997 to 2005,8 cases of coronary artery fistula(all proved by operation) in our hospital together with 145 cases reported in recent nucleus joumals,a total of 153 cases coronary artery fistula color Doppler two-dhmenstonal ultrasonic echocardiogram characteristics were analyzed and found the rules of blood dynamics and characteristic of pictures in coronary artery fistula. Results According to characteristics of picture, color ultrasonic echocardiogram could diognose 133 cases accurately. Accuracy of diagnesis reached 87 %. Two-dimenstonal ultrasonic echocardiogram and color Doppler, echocardiogram and characteristics of spectrum were conspicuous and showed coronary orifice and internal-diameter of involved coronary artery ,followed the direction of fistula,observed which atrium or ventricle perforated and guided the operation of intervention therapy. Conclusion Color Dopper two dimentsional ultrasonic echocardiogram had diagnostic value in coronery artery fistula. It is the first choice of diagnosis.
