摘要
目的分析苯唑西林和万古霉素对金黄色葡萄球菌的纸片扩散法抑菌圈直径和E-test法最低抑菌浓度间的关系。方法用纸片扩散法检测苯唑西林和万古霉素对金黄色葡萄球菌的抑菌圈直径,用E-test法检测最低抑菌浓度(M IC)。结果纸片扩散法和E-test法检测金黄色葡萄球菌对苯唑西林的敏感性,敏感率分别为85.0%和86.5%,中介率分别为2.3%和3.0%,耐药率分别是12.7%和10.5%,个别经K irby-Bauer法判定的中介株或耐药株经E-test法M IC证实为敏感或中介株;所有的菌株都对万古霉素的敏感,其中抑菌圈直径≥15 mm的菌株占91.7%。结论用K irby-Bauer法和E-test法检测金黄色葡萄球菌对万古霉素和苯唑西林的敏感性,其药敏结果基本一致,但纸片扩散法结果处折点值附近或万古霉素对菌株的抑菌圈低于超出敏感值下限时,应测量相应抗生素的最低抑菌浓度。
Objective: To analyze the relations between the zone diameters and minimal inhibitory concentrations (MICs) of oxacillin and vancomycin against staphylococcus aureus in susceptibility test in vitro. Methods: The susceptibility of oxacillin and vancomycin against staphylococcus aureus were performed by both disc diffusion method and E-test method simultaneously in vitro, and both the zone diameters and MICs were measured by the same operator. Results: Of all 133 strains performed by both disc diffusion and E-test methods in the susceptibility test, 85.0% and 86.5% of the isolates were oxacillin-susceptible,while 12.7% and 10.5% were oxacillin-resistant respectively. Several isolates which could not be clearly distinguished by the method of Kirby-Bauer were determined as susceptible or between susceptible and resistant by the method of E-test. All of the isolates were susceptible to vancomycin with E-test MICs ( concentrations〈4.0 μg/ml) and 91.7% of them had the zone diameters more than 15 mm. Conclusion: The E-test and Kirby-Bauer method appeared to be comparable in identifying susceptible or resistant strains, and all isolates with zone diameters near breakpoints or the zone diameters of vancomycin to staphylococcus aureus less than 15 mm should be tested by an MIC method.
