摘要
手术治疗老年腰椎间盘突出症３２例，术前均有不同程度的下肢痛，１０例有间歇性跛行．３０例经脊髓造影、ＣＴ或ＭＲＩ确诊．行半椎板切除减压术５例，全椎板切除减压术２７例．术中见有２个间隙突出的８例，中央型和正中偏旁型２８个，Ｌ_（３～４）４个，Ｌ_（４～５）２１个，Ｌ_５-Ｓ_１１５个．随访２９例，满意率９３．７％．主张对于患腰腿痛的老年人，应进一步检查有无椎间盘实出，一旦确诊应尽早手术．手术以全椎板切除＋关节突内缘＋椎体后缘增生的骨赘和椎间盘组织一起切除为佳．
This article reports 32 cases of elderly after operation for prolapse of the lutnbar intervertebral disc.Before operation,all the patients had a pain of lower extremity and 10of them intermittent claudication.Myelography,CT or MRI study led to confirmed diagnosis.The total laminectomy with decompression was performed on 27 cases,and semilaminectomy with decompression on 5cases.Double prolapse of intervertebral disc was found in 8 cases during operation,Twenty-eight prolapses were of central or paramedial type,among which 4 were located at L_(3～4),21 at L_(4～5),and 15 at L_5S_1.Twenty nine cases received follow-up with a satisfactory rate of 93.7%.The authors suggest that a careful examination for intervertebral disc must be carried out on the elderly suffering from lumbodynia.Operation should be done as soon as the diagnosis is established.It is commended to performe total laminectomy with resection of intervertebral disc,medial portion of the articular process and the vertebral osteophyte.
