摘要
内植物断裂是脊柱侧弯术后晚期并发症之一，常导致侧弯矫正率明显丢失。作者就１７７例脊柱侧弯矫正患者的断棒原因（断棒率１０．７％）进行多方面探讨：（１）通过对棒体材料组分的定性定量分析及断面形态的电镜观察表明：金属材料中，铬、镍、锰、钼、硅、磷含量均呈不同程度异常，材质的热处理态使组织晶粒较粗呈脆性，在使用中环境介质具腐蚀性，且外载为周期性加荷，因此认为材料断裂为应力腐蚀疲劳所致；（２）材料的外形设计与受力状态对材料强度影响较大，表现在哈氏棒断裂部位均发生在棒节交界区及所有断棒在术前进行过不同程度的预弯处理；（３）脊柱结构异常时，棒体所受的复合应力作用随侧弯程度的加剧而增大，从而加速了棒体的破坏过程。同时强调良好的骨融合是确保脊柱永久性稳定的关键，而术后长期有效的外固定是完成骨融合的必要条件。
Theimplantfracture,oneofthelatecomplicationsfolowingtheinstrumentationforsco-liosis,frequentlyleadstothelossofcorrection.Thisstudyanalysedthecausesofimplantfractureus-ingthequalitiatveandquantitativeanalysesofthecompsitionoftheimplantmaterial,andtheultrastru-cturalobservationofthesectionofthebrokenimplants.Theresultsmaybeconclusedasfolowing:(1)Themajorcauseofthematerialfracturewasthestressfatigueandmetalcorosion;(2)Theconfigu-rationdesignoftheimplantsandthecontributionofloadhadastrongimpactonthematerialstrength;(3)withabnormalstructureofthespine,thecompoundstressontheimplantsincreasedwiththein-creaseoftheseverityofscoliosis.Itshouldbeparticularlyemphasizedthatasolidspinalfusionisessen-tialtotheperminantstabilityofthespineandtheefectiveandlongtermexternalfixationisnecessarytoachiveasolidfusion.
