摘要
[目的]研究治疗肱骨近端严重粉碎性骨折的一种新方法.[方法]分析自2003年1月～2005年1月收治的8例肱骨近端严重粉碎性骨折患者的临床资料,通过患者术后肩关节功能恢复情况,评价Ender氏针加钢丝张力带内固定治疗肱骨近端严重粉碎性骨折的疗效.[结果]本组8例患者均获随访,随访时间6～18个月,平均11个月.采用Neer评分法评定疗效,其中优4例,良3例,可1例,差0例.[结论]Ender氏针加钢丝张力带内固定治疗肱骨近端严重粉碎性骨折具有重建肩关节解剖关系,使骨折达到功能复位和牢固内固定、术后不需外固定的优点,有利于术后早期功能锻炼,恢复肩关节功能.
[Objective]To research a new method to treat severely comminuted proximal humeral fractures. [Method] From January 2003 to January 2005, 8 patients with severely comminuted proximal humeral fractures were admitted to our hospital. All patients underwent the operation of open reduction and internal fixation with Ender nail and tension band. The effect of the operation through the recovery of the shoulder after the operation was evaluated. [ Result] All the 8 cases were followed up. The followup time ranged from 6 to 18 months with an average of 11 months. According to Neer scoring system, excellent result was 4 cases, good was 3 cases and fair was 1 case. No case was poor. [ Conclusion ] Internal fixation with Ender nail and tension band could treat severely comminuted proximal humeral fractures has the advantages of reconstructing the normal anatomy of shoulder joint, obtaining the functional reduction and rigid internal fixation, no need for external fixation after operation and being helpful to earlier training and to recover functions of the disordered shoulder.
