摘要
目的总结成人继发孔房间隔缺损合并肺动脉高压的外科治疗和围手术期处理经验。方法1996年1月至2005年3月,手术治疗合并肺动脉高压的成人继发孔房间隔缺损46例,其中男性19例,女性27例,年龄18～54(31.5±4.2)岁。所有患者均行房缺修补术,其中补片修补30例。根据具体病情,围术期给予吸氧、高压氧及前列腺素E1等治疗。结果无手术死亡,术后发生低心排综合征9例,右心功能不全5例,心律失常21例,肺部感染6例。随访32例,随访时间3个月至6年,心功能状态良好。结论成人房间隔缺损应尽早手术治疗。对于合并肺动脉高压的患者,合理的围术期处理可以降低手术死亡率和并发症的发生。
Objective To summarize the experiences of surgical treatment of secondary atrial septal defects combined with pulmonary hypertension in adults. Methods A total of 46 patients with secondary atrial septal defects and pulmonary hypertension were treated surgically in our hospital from Jan 1996 to Mar 2005. The patients included 19 male and 27 female and their age ranged from 18 to 54 （mean 31.5 ±4.2） years. Surgical procedures of closing defects were performed in all the patients. Surgical closure of 30 cases was done by patches, including 16 autologous pericardial patches. All patients were treated perioperatively with oxygen inhalation therapy, hyperbaric oxygenation, prostaglandin El respectively according to the degree of pulmonary hypertension. Results There was no death during operation. After the procedures, 9 cases occurred low cardiac output syndrome, 5 cases right ventricular amyoplasia, 21 cases arrhythmia, and 6 cases pneumonic infection. Totally 32 cases were followed up from 3 months to 6 years. The heart function were all in better status. Conclusion Atrial septal defects in adult should be corrected as early as possible. Proper perioperative management could reduce postoperative mortality and morbility for adult atrial septal defects with pulmonary hypertension.
