摘要
目的研究子宫颈人乳头状瘤病毒16、18型(HPV16·18型)感染与子宫颈癌及癌前病变的关系。方法我院妇产科宫颈疾病筛查中心对2003年1月～2005年6月求诊的20至60岁的已婚妇女6025例进行宫颈疾病诊疗,应用Taqman荧光PCR法定性检测宫颈分泌物HPV16·18型DNA,然后对HPV16·18型DNA阳性的妇女行阴道镜检查及宫颈组织活检病理检查。结果3550例HPV16·18型DNA阳性,其中慢性宫颈炎2628例(74.1%),子宫颈癌前病变(CINⅠ～Ⅱ级)874例(24.6%),子宫颈癌(包括原位癌)48例(1.4%)。慢性宫颈炎HPV16·18型DNA含量为115±20,子宫颈癌前病变HPV16·18型DNA含量为323±15,子宫颈癌的HPV16·18型DNA含量为625±57。结论Taqman荧光PCR方法可作为检测子宫颈HPV16·18型感染的手段,且可作为宫颈癌前筛查除病理学之外较可靠的检查方法。
Objective To study the relationship between loads of human papillomavirus-16 · 18 and cervical carcinoma and cervical intraepithelial neoplasia. Methods From Jan 2003 to Jun 2005, six thousand and twenty-five married women aged from 20 to 60 years were screened for high risk types of human papillomavirus （HPV） infection by using Taqman-PCR. Then the women with positive human papillomavirus underwent colposcopy and directed punch biopsies. Results There were 3550 women with positive human papillomavirus. Among them there were 2628 （74. 1%） women with cervicitis, 874 （24.6%） women with cervical intraepithelial neoplasia and 48 （1.4%） women with cervical squamous cell carcinoma. HPV-16 · 18-DNA loads of women with chronic cervicitis were 115±20, those of women with cervical intraepithelial neoplasia were 323±15, and those of women with cervical squamous cell carcinoma were 625±57. Conclusion The new method of Taqman-PCR is an effective technology to detect HPV infection in cervix. It could be used for screening primary cervical carcinoma.
