摘要
目的测定高血压及合并糖尿病患者心率的变异性情况。方法３８例高血压患者，１８例高血压合并糖尿病患者和１８例健康对照者，用动态心电图仪测定２４ｈＲＲ间期的变异性（ＨＲＶ）。结果心率变异性在试验对象明显降低，尤其合并有糖尿病者。随访发现心血管事件的发生率在试验组为１６．７％，而健康组为０；心血管事件组的ＨＲＶ明显低于无心血管事件组。以ＨＲＶ的３种指标而论，发生心血管事件组都明显高于无发生心血管事件组３种指标，预测心血管事件的敏感性为５５％～６６％，特异性为８２％～９５％，准确性为７８％～９１％。
Aim\ To investigate the heart rate variability in patients with hypertension and hypertension complicated with diabetes.\ Methods\ By using Holter 24 h R R intervals variability analysis system, the heart rate variability(HRV) was measured in 38 patients with essential hypertension(EH). 18 patients with essential hypertension complicated with diabetes(EHCD) and 24 normal controls(NC) were compared.\ Results\ The HRV of patients with EH and EHCD is markedly depressed,being more remarkable in the later group. Follow up investigation found that the incidence of cardiovascular events(CVE) is 16.07% in EH and EHCD group, whereas without any cases was found in NC group. The HRV of CVE group is obviously lower than non CVE group. Regarding the three parameters of HRV, the positive rates of them in CVE group are higher than those of non CVE group,the sensitivity for predicting CVE is 55.55%￣66.66%, the specificity is 82.97%～95.74%,the accuracy is 78.57%～9 07%.\ Conclusion\ The decreased HRV correlate to CVE and may have important value for assessing prognosis of the patients with EH and EHCD.
