摘要
目的:报道20例尺侧腕关节慢性疼痛的治疗结果,并探讨其与三角纤维软骨损伤的关系。方法:通过对临床症状、X线片表现、腕关节造影11例、腕关节镜检查10例、12例手术发现及治疗结果等资料的分析,研究其病因,诊断和预后。结果:三角纤维软骨损伤11例,慢性滑膜炎2例,软骨骨折1例,尺月韧带部分撕裂1例;无肯定诊断5例。随访6个月以上,发现疗效为优者13例(无痛并恢复原来工作);良者4例(用力时轻度疼痛,恢复原来工作);差者3例(仍有持续性痛)。13例腕关节检查时表现有卡嗒声者,其中10例为三角软骨损伤。结论:当检查发现尺侧腕关节有痛性卡嗒声且症状持续在3个月以上者,则很可能为三角纤维软骨损伤。
Objective: To report the treatment of 20 cases of chronic ulnar wrist pain and to investigate its relationship with triangular fibrocartilage injury. Methods: On the bases of clinical presentations, x-ray, surgical findings and treatment results, the etiology, diagnosis and prognosis were studied. The operative procedures performed included arthrogram in 11 wrists, arthroscopy in 10 wrists and surgical exploration in 12 wrists. Results: Triangular fibrocartilage was found to be injured in 11 wrists. Chronic post-traumatic synovitis was diagnosed in 2 wrists. One wrist had chondral fracture and one had partial tear of ulno-lunate ligament. 5 wrists had no definite diagnosis. After follow up for more than 6 months, 13 patients had excellent, 4 had good and 3 had poor end results. Conclusions: Analysis of the results showed that painful click over the ulnar side on examination had high association with triangular fibrocartilage injury, when the symptoms lasted for more than 3 months.
