摘要
利用小腿内踝上４～６ｃｍ处胫后动脉穿枝血管为皮瓣营养蒂，设计小腿内侧逆行筋膜皮瓣，皮瓣大小可达１４ｃｍ×１０ｃｍ。皮瓣应用２６例。皮瓣转移形式为岛状，交腿，窄蒂状，修复了小腿中下段部位的创面，骨外露以及足跟足底创面。结果：皮瓣完全成活２３例，部分坏死３例。应注意岛状形式转移皮瓣时，皮瓣不宜过大，小腿内侧逆行筋膜皮瓣切取时不必解剖及切断胫后功脉，因而对足部血运无影响。
According to anatomical dissection,large fasciocutaneous perforator was located 4～6cm above the medial malleolus on the posterior tibial border,The flap was designed as large as 14 by 10cm. Clinical uses include cover of lower half of the tibia foot.The flap has been used in 42 cases with only 3 partial failures.The cross leg flap,island flap,and narrowed padicle flap were used for flap trans-fe.The distally based fasciocutaneous flap of the medial leg is useful and the dissection of the flap is quick and simple without sacrificing the posterior tibial artery.
