摘要
为探讨光动力学疗法对消化道癌术后局部复发癌的效果，对此类患者１０３例应用光动力学疗法。患者于照光前４８～７２小时静脉滴注ＨｐＤ，每公斤体重５ｍｇ。激发光源采用氩离子泵浦染料激光器，波长６３０ｎｍ，输出功率密度２００～４００ｍＷ／ｃｍ２，能量密度２００～２５０Ｊ／ｃｍ２，照射时间约１０～１５分钟。１０３名患者中，食管癌１４例、贲门癌５０例、胃癌３３例、直肠癌６例。有效者７４例，总有效率７１．８％。肿瘤小于１ｃｍ者３６例，３４例有效，有效率９４．４％。随访存活１年以上者３６．９％，２年以上者１８．４％，２例最长存活者已１０年尚健在。
mg/kg of HpD was given intravenously 48～72h prior to PDT. The light sourse was an argon pumped dye laser with a wavelength of 630nm. The irradiation time varied from 10～15 min with a power density of 200～400 mW/cm 2. The energy density was 200～250 J/cm 2. Of the 103 patients, 14 had cancer of the esophagus, 50 had cancer of the gastric cardia, 33 had cancer of the stomach and 6 had cancer of the rectum. 74 patients (71.8%) had a complete or significant response to PDT. There were 34 responders out of 36 patients (94.4%) with tumour size less than 10mm. The following survey of all patients lasted from six months to ten years. 28 of 103 patients (36.9%) survived more than one year, 18.5% more than two years, two patients have been followed up for more than 10 years without signs of recurrence and are still being followed up.
