摘要
目的探讨电视胸腔镜(VATS)下微创手术治疗食管癌的适应证及操作特点.方法回顾分析在VATS下完成13例Ⅰ~Ⅲ期食管中、下段癌手术中操作方法、手术效果及手术后并发症.结果成功完成12例VATS下食管癌手术,1例辅助切口完成手术.VATS平均操作时间90min,手术平均出血225 ml.共清除淋巴结95个.术后并发乳糜胸1例,吻合口漏1例,无手术死亡.随访4~14个月,患者全部健在.结论VATS下微创手术治疗食管癌,手术创伤明显减轻,为高龄、肺功能差的患者增加了手术机会,也为早期食管癌患者缩短了术后恢复时间.
Objective To investigate the indication and procedure of esophagectomy under thoracoscopy(VATS). Methods Thirteen esophagectomies were performed under thoracoscopy in patients with Ⅰ to Ⅲ stage esophageal cancer. The outcomes and postoperative complications were analyzed retrospectively. Results VATS was carried out successfully in 12 patients, and combined with a small thoracic incision in one case. Mean duration of the procedure was 90 min with an average bleeding of 225 ml. 95 lymph nodes were resected. One patient was complicated with chylothorax, and anastmosis leakage happened in one case. The patients were followed up for 4 to 14 months postoperatirely,and all 13 patients were alive well. Conclusion Esophagectomy can be performed successfally under thoracoscopy with less surgical trauma. VATS may provide the aged esophageal cancer patients of poor lung function with surgical resection opportunity , and shorten the time of hospital stay in patients with early stage esophageal cancer.
