摘要 目的:探讨胶质纤维酸性蛋白在缺血性及创伤性脑损伤、脑水肿等中枢神经系统疾患时表达的变化及所起的作用。资料来源:利用计算机检索Medline 1998-01/2005-03有关缺血性脑损伤时胶质细胞纤维酸性蛋白表达与作用的文献,检索词“Glial fibrillary acidic protein”,限定文献类型为English。同时检索CBMdisc1998-01/2005-03有关缺血性脑损伤时胶质细胞纤维酸性蛋白表达与作用的文献,检索词为“神经胶质原纤维酸性蛋白质”。资料选择:Medline检索到的文献共280余篇,通过浏览文题和摘要,选择与脑缺血性损伤、创伤性脑损伤和脑水肿方面的文献,查找原文阅读。国内文献CBMdisc检索到摘要性文献294篇,同样通过CNKI检索系统查找关于上述要点的文章全文。资料提炼:检索关于胶质纤维酸性蛋白在缺血性脑损伤时表达与作用的文献共21篇,选择观点相似的文献引用11篇;并检索到5篇关于创伤性脑损伤和脑水肿时血液中胶质纤维酸性蛋白变化的文献。资料综合:作为胶质细胞增多症和星形胶质细胞激活的标志物,胶质纤维酸性蛋白在缺血性脑卒中时脑组织和血液中表达明显增强,缺少它的星形胶质细胞被证实在突触形成上、血脑屏障的诱导和容量调控方面有缺陷,更容易受到脑缺血的损伤。另外,胶质纤维酸性蛋白在创伤性脑损伤及脑水肿时血清含量亦有不同程度的增高,但此蛋白对前者来讲似乎是脑损伤的早期指标并可反映损伤的严重程度;对于脑水肿来讲,虽然血液中有不同程度的升高,但同疾病的严重程度并无关系。结论:胶质纤维酸性蛋白在缺血性脑损伤的脑组织中表达增强,并且有对抗缺血性损.伤的作用,血液中含量的增加可能作为缺血性脑卒中和创伤性脑损伤早期评估、揭示疾病严重程度的指标。脑水肿时胶质纤维酸性蛋白含量也增加,但似乎同疾病的严重程度并无关系。 OBJECTIVE: To discuss the expression changes and role of glial fibrillary acidic protein (GFAP) in central nervous system diseases, such as cerebral ischemia, traumatic brain damage and encephaledema. DATA SOURCES: We searched the Medline for articles on expression changes and role of GFAP after ischemia brain injury published from January 1998 to March 2005, with the key words of “Glial fibrillary acidic protein” in English. Meanwhile, articles on expression changes and role of GFAP after ischemia brain injury were searched in CBMdisc published from January 1998 to March 2005, using the same key words. STUDY SELECTION: More than 280 literatures were retrieved through Medline. Titles and abstracts were browsed, and literatures about cerebral ischemia damage, traumatic cerebral damage and encephaledema were selected and full text were got and read. 294 domestic literatures with only abstracts were retrieved through CBMdisc, and full texts of the abovementioned articles were got through CNKI system. DATA EXTRACTION: There were 21 literatures about expression and role of GFAP after ischemia brain injury, and 11 articles that had representing viewpoints were cited; 5 articles were available on the topic of changes of blood GFAP level in patients undergoing traumatic brain damage and eneephaledema. DATA SYNTHESIS: As the marker of glialcytosis and activation of astrocytes, the expression of GFAP in brain tissue and blood significantly increased in patients undergoing ischemia stroke, and astrocytes lacking GFAP had been proved to he defective in process formation, induction of hlood-brain barrier and volume regulation, and were more vulnerable to cerebral ischemia damage. Otherwise, the level of GFAP also elevated to some degree in serum of traumatic brain damage and encephaledema patients. But to cerebral iscbemia patients, this protein seemed to be an early marker of brain damage and could reflect the severity of this damage; To encephaledema patients, although the elevation could also be observed to some degree, but there was no close relationship between their level of GFAP and severity of disease. CONCLUSION: The expression of GFAP in brain tissue undergoing ischemia cerebral damage significantly increases, which Can counteract the ischemia cerebral damage. The elevation of GFAP level can be an index that reflects early damage and severity of ischemia stroke and traumatic brain damage. Although this kind of elevation of GFAP can also be observed in encephaledema patients, there was no close relationship between the level of GFAP and severity of disease.
