颅眶沟通性肿瘤的MRI诊断(附18例报告)

摘要 目的探讨颅眶沟通性肿瘤的MRI表现特征与诊断依据。方法经手术病理证实的颅眶沟通性肿瘤18例,行MRI横轴位、矢状位、冠状位与增强扫描。结果脑膜瘤10例,T1WI等信号或略低信号,T2WI等信号或略低信号,肿块明显均匀强化,常合并蝶骨增生。视神经鞘脑膜瘤3例,视神经不规则管状或梭形增粗,T1WI等信号,T2WI为等或略低信号,增强后呈“双轨征”。视神经胶质瘤1例,眶内见纺锤形肿块,T1WI等信号,T2WI高信号,肿块明显强化呈“哑铃征”。炎性假瘤3例,T1WI低信号,T2WI低信号,肿块明显强化,眼外肌及眼睑常明显强化。黄色肉芽肿1例,T1WI等信号,T2WI低信号,眼外肌明显增粗,小脑幕对称性等T1短T2信号,病灶明显均匀强化。结论MRI在诊断颅眶沟通性肿瘤具有明显的优势,可以提高颅眶沟通性肿瘤的诊断水平,为临床治疗提供可靠依据。 Objective To investigate the MRI characteristics and diagnostic value of craniorbital tumors. Methods Eighteen cases of craniorbital tumors were confirmed by pathology, which were scanned with transverse, sagittal, coronal and contrast enhanced scanning. Results There were ten cases of meningioma, which were isointense or slightly hypointense on T1 weighted images (T1WI), isointense and slightly hypointense on T2-weighted images (T2WI), with marked uniform enhancement. Hyperostotic changes of the sphenoid bone were also found frequently. There were 3 cases of meningioma of optic nerve sheath, optic nerves were fusiformly thickened or tubularly enlarged, which were isointense on T1WI, isointense and slightly hypointense on T2WI. The "tram-track" sign was clearly demonstrated after contrast-enhanced scanning. There was one case of optic gliomas, the spindle shaped mass was shown in orbit, which was isointense on TlWl, hyperintense on T2WI: the "dumh bell" sign was clearly demonstrated after enhancement. There were3 cases of orbital inflammatorypseud otumor, which were hypointense on TlWl, hypointense on T2WI, with marked enhancement; the muscle and eyelid were often greatly enhanced. There was one case of xanthogranuloma, which was isointense on TlWl, hypointense on T2WI; hypertrophy of the muscle of eye was shown evidently; isointensity signal on TlWl and hypointensity signal on T2WI were found symmetrically in the bilateral tentorium of cerebellum, with strong uniform enhancement. Conclusion MRI is valuable in the diagnosis of craniorbital tumors, which will improve diagnostic level and provide a scientific basis for clinical treatment.
